基于湿热虫邪探讨幽门螺杆菌感染的中医证治

基于临床观察,结合取象比类法,幽门螺杆菌(Hp)可被视为具有湿热性质的虫邪,即湿热虫邪.Hp感染以湿热虫邪侵袭胃腑为发病关键,脾胃虚弱或脾胃湿热状态为湿热虫邪的侵袭提供了内在条件.湿热虫邪定植后即会影响胃腑气血运行,产生虫毒损害人体,初期病邪轻浅,机体尚可耐受,侵袭日久病邪深入,变证由生.湿热虫邪由浅入深,由影响无形之气到有形之邪内生,此过程可见有虫邪郁滞、胃气失和,邪郁中焦、酝而化热,气血失和、虫毒相兼,脾胃内伤、气阴亏耗的病机演变过程,分别以和降胃气、清热除湿、活血解毒、平补脾胃为治法.
TCM syndrome differentiation and treatment of helicobacter pylori infection based on damp heat insect pathogen
On the basis of clinical observation combined with analogism,helicobacter pylori can be classified into an insect patho⁃gen with damp heat properties,namely damp heat insect pathogen. The key to the onset of helicobacter pylori infection is the invasion of damp heat insect pathogen into the stomach,and the weakness of spleen and stomach or damp heat of spleen and stomach provide internal conditions for the invasion of damp heat insect pathogen. After the colonization of damp heat insect pathogen,it will affect the circulation of qi and blood in the stomach and produce insect toxins that damage the human body. In the initial stage,the patho⁃gen is mild and shallow,the body can still tolerate it. Over time,the disease pathogens get deepened. The pathogenic process of dampness and heat,from the shallow to the deep and from the influence of intangible qi to the endogenous generation of tangible evil,can be seen in the stagnation of insect evil,the disharmony of stomach qi,the stagnation of evil in the stomach,the brewing of heat,the disharmony of qi and blood,and the combination of insect and toxin,as well as the internal injury of the spleen and stomach and the depletion of qi and yin. The treatment principles of descending stomach qi,clearing heat and dampness,promoting blood circulation and detoxification,and tonifying the spleen and stomach are adopted respectively.
