基于象思维从"虫"论治小儿过敏性疾病
Treatment of pediatric allergic diseases using image thinking through the lens of the chong theory

摘要
象思维是中医学理论体系的基本思维模式之一,临证中运用象思维辨治疾病,发现过敏性鼻炎、过敏性咳嗽、支气管哮喘、湿疹等过敏性疾病的发病特点与"时发时止、反复发作、上下窜动、动静多变"的"虫象"相似,透过其形色各异的症状识别其虫象特征,尝试从虫论治:湿热生虫,虫邪蛰伏体内,成为过敏性疾病之宿根;风来虫动、气机逆乱为其主要病机.基于虫象以乌梅丸加减治疗有较好疗效.文章运用象思维对从虫论治小儿过敏性疾病的病因病机和治法方药进行系统阐述,并列举验案加以说明,为临床治疗小儿常见过敏性疾病提供新的辨治思路和有益借鉴.
Abstract
Image thinking,or analogical thinking,constitutes a fundamental cognitive approach in the theoretical framework of traditional Chinese medicine.In clinical practice,we utilize this thinking mode to identify diseases.We have observed that the characteristics of allergic diseases such as allergic rhinitis,allergic cough,bronchial asthma,and eczema resemble the pattern of chong(worm),which exhibits traits of intermittent onset,recurring episodes,dynamic fluctuations,variability in symptoms.By identifying the distinct chong-like features through diverse symptom manifestations,we attempt a therapeutic approach based on the chong theory:attributing the origin of allergic diseases to the generation of worm by dampness and heat and the infiltration of dormant chong within the body,constituting the perennial root of allergic diseases.Moreover,the increased activity of chong induced by external wind disrupts the movement of qi,which is the main pathogenesis of such conditions.Employing Wumei Pill(Mume Pill)based on the chong theory has yielded notable therapeutic efficacy.In this paper,the etiology,pathogenesis and treatment method,and specific herbal remedies for children's allergic diseases through the lens of the chong theory are systematically expounded via image thinking.Additionally,illustrative cases are presented to provide novel treatment perspectives and guidelines for managing common allergic diseases in pediatric clinical settings.
