摘要
多系统萎缩是一种原因不明的散发性、致死性的神经系统变性病.王永炎院士提出"虚气留滞"理论,"虚气"指元气亏虚,即肾元虚损,为发病之本,"留滞"指留滞邪气,即湿聚、痰结、血瘀,为病变之标."虚气"影响津血运行,形成"留滞","留滞"盘踞体内,损伤"虚气","留滞"盛而"虚气"衰是多系统萎缩病情进展的关键.临证治疗基于"虚气留滞"理论进行分期论治,平稳期肾元虚损为核心,强调补肾培元,固其根本;病进期虚滞并存,着重祛邪消滞,采取祛湿、化痰、活血等法,缓其虚损.持续治疗,两期兼顾,为治病之宗旨.
Abstract
Multiple system atrophy(MSA)stands as a sporadic,fatal neurodegenerative disease with an obscure etiology.Prof.Wang Yongyan,Academician of CAE,proposed the theory of"stagnation of deficiency qi".Here,"deficiency qi"refers to the depletion of original qi,specifically the deficiency within the kidney,which is the basis of the disease.Meanwhile,"stagnation"pertains to the retention of evil qi,characterized by the accumulation of dampness,phlegm,and blood stasis,representing pathological manifestations.The impact of deficiency qi disrupts the circulation of blood and fluids,fostering the formation of"retention and stagnation",which,once entrenched,impairs the deficiency qi.The progression of MSA hinges crucially upon the coexistence of surging"stagnation"and the waning of"deficiency qi".Clinical intervention,underpinned by this theory,adopts a staged approach.During the stable phase,the focus revolves around the depletion of kidney original qi,emphasizing the strengthening of the kidney and strengthening its foundation.In the progressive stage,where deficiency and stagnation coexist,emphasis is laid on eliminating pathogenic factors and resolving stagnation.Strategies encompass removing dampness,resolving phlegm,and activating blood to decelerate the progression of deficiency and damage.A treatment approach that encompasses both phases and continuous therapeutic measures epitomizes the fundamental principle of MSA management.
