摘要

化学治疗(简称"化疗")是目前肿瘤治疗的重要手段,化疗后所导致的周围神经病变(CIPN)是临床常见不良反应.肾虚及相关化疗药物治疗是CIPN发病的始因,瘀血阻络是CIPN发病的核心环节,肾虚络瘀是CIPN的基本病机.临床以"久病必虚,久病必瘀,久病及肾,久病入络"为理论基础,以辨病与辨证论治相结合为基本原则,以益肾通络为治法,运用益肾解毒通络汤治疗该病.并结合临床医案对运用益肾通络法治疗CIPN的经验进行介绍.

Abstract

Chemotherapy stands as a pivotal approach of tumor treatment.Chemotherapy-induced peripheral neuropathy(CIPN)constitutes a prevalent adverse reaction in clinical practice.Kidney deficiency and the use of chemotherapeutic drugs serve as the primary triggers of CIPN.The core element in the onset of CIPN is blood stasis.Kidney deficiency and blood stasis is the fundamental pathogenesis of CIPN.Guided by the principle that"long-standing illness leads to deficiency and stasis,affects the kidney,impacts the collaterals",our clinical approach combines disease differentiation and evidence-based treatment.Employing the method of boosting the kidney and clearing the collaterals,we use Yishen Jiedu Tongluo Decoction(Kidney-Boosting Toxin-Removing and Collateral-Clearing Decoction)to treat this condition.Additionally,this paper introduces clinical cases to share experiences in treating CIPN with the method of boosting the kidney and clearing the collaterals.
