Pulmonary nodules manifest as focal,circular-shaped and increased density of solid or subsolid lung shadows with a diameter of ≤3 cm,classified under the categories of feiji(pulmonary accumulation),jiju(abdominal mass)and xiben(hypochondrium lumps)in traditional Chinese medicine.Professor Wang Qingguo,laureate of the title of National TCM Master,put forward the distinctive diagnosis approach of"simultaneous treatment of the disease,syndrome,symptom and tendency",and the idea of"prioritizing disease differentiation as the primary step,syndrome differentiation as the main approach,complemented by symptom differentiation,with the knack of identifying the tendency".In the first step,a main prescription is chosen.It is believed that the core pathogenesis of pulmonary nodules is qi stagnation,phlegm obstruction,resulting in solid accumulation in the lung,which is specifically addressed with Wangshi Kaijie Prescription(Wang's Nodule-Resolving Prescription).Furthermore,syndrome differentiation assumes a pivotal role in treatment based on the pathological mechanism.It is believed that patients with pulmonary nodules are primarily categorized into three groups:those predominantly characterized by deficiency of healthy qi,excess of pathogenic factors,or a presentation of both.Tailored prescriptions are given accordingly.Symptoms differentiation supplements this approach by addressing specific symptoms and adjusting the formula accordingly to alleviate urgency.The method of differentiating tendency is strategic in averting disease evolution.Clinical attention significance is placed on"preventative treatment based on the tendency",wherein proactive administration of corresponding herbal medicine is employed to preempt the progression of certain pulmonary nodules into lung cancer.Here,a proven case record is provided to elucidate Professor Wang's clinical rationale and experience.
