摘要
银屑病是一种慢性、复发性、炎症性皮肤病,其诱发因素众多、机制复杂,反复发作,难以治愈,对患者的生活质量有较大影响并造成一定的心理负担.钱文燕教授认为银屑病的病理因素主要有风、热、瘀、毒;从物质层面而言涉及气血水,以血分为主;主要病机包括血燥、血热、血瘀3类;在疾病的发展过程中,情志因素为影响疾病向愈的关键因素.遣药组方注重以解毒药为主导,针对不同病因及不同症状灵活选用,清热而不涩气机、凉血而不使凝滞,强调应给邪以出路;治疗注重身心同调,多用轻清疏气、解郁安神之法;并配合外用洗方及恰当的西药,内外合治、标本兼顾,常收良效.通过临床病案介绍钱教授辨治银屑病的经验.
Abstract
Psoriasis is a chronic,recurrent and inflammatory skin disease,which has many predisposing factors and complex mechanisms,involving multiple systems,recurrent and difficult to cure.It has a great impact on the quality of life of patients and induces psychological burdens.Professor Qian Wenyan identifies the core pathogenesis of psoriasis as wind,heat,blood stasis and toxin.From the material level,it involves qi,blood and water,and mainly includes blood dryness,blood heat and blood stasis.In the process of disease progression,emotional factors are the key factors affecting the recovery of patients.In response,Professor Qian's treatment approach focuses on leading with toxin-removing herbs,adapting prescriptions to various causes and symptoms.The emphasis lies on clearing heat without astringing qi movement,and facilitating the expulsion of pathogenic factors.Furthermore,it is crucial to maintain harmony between body and mind,and utilizing the method of clearing qi and relieving constraint.Complemented by external washing prescription and appropriate western medicine,the integrated internal and external treatment approach,addressing both symptoms and underlying causes,consistently yields positive outcomes.This paper presents her experiences in the diagnosis and treatment of psoriasis through the exploration of a case study.
