摘要
"中气"理论是《四圣心源》的核心理论,黄元御认为中气乃脾胃升降的枢纽,中气健运则升降相宜,四维运转,中气不运则升降失司,百病乃生,治疗时当首重中气,此理论广泛运用于反胃、噎膈、鼓胀、便秘、泄泻、痢疾及吐血、便血等多种消化系统疾病之中,这对后世临床产生了深远的影响.本文基于此从中气与四维的关系、中气与气血化生的关系及中气与水湿形成的关系三个方面探讨其病机认识.通过收集整理相关文献,基于诸多医家对"中气"理论在消化系统疾病中的临床应用,总结出以扶阳抑阴为治疗大法,以健运中气为基础,注重调节四维运转、气血运行,同时注重协调肝脾关系的治疗特色,为临床提供新的参考依据.
Abstract
The theory of zhongqi(center qi),a core concept of Si Sheng Xin Yuan(The Original Spirit of the Four Sages)as elucidated by Huang Yuanyu,posits that Zhongqi serves as the pivot for the ascent and descent of the spleen and stomach.When zhongqi is robust and operates harmoniously,the four elemental processes unfold seamlessly;however,if zhongqi falters,disruptions in ascending and descending functions occur,giving rise to various ailments.In therapeutic interventions,prioritizing zhongqi becomes paramount.This theory is widely used in addressing diverse digestive system maladies such as nausea,dysphagia,bloating,constipation,diarrhea,dysentery,hematemesis and hematochezia,which has a profound impact on later clinical practice.This paper delves into the pathogenesis from three aspects:the relationship between zhongqi and the four elements,the relationship between zhongqi and qi-blood transformation,and the relationship between zhongqi and water-dampness formation.By collecting and sorting out the relevant literature and drawing on the clinical applications of the zhognqi theory in digestive system diseases by various doctors,this paper identifies the treatment emphasis on strengthening yang and suppressing yin.This approach,rooted in the robust functioning of zhongqi,emphasizes the regulation of the four-element operations and the circulation of qi and blood.Simultaneously,it underscores the importance of harmonizing the relationship between the liver and spleen.The findings aim to provide fresh perspectives for clinical practitioners.
