摘要
石膏临床广泛应用于多种疾病,生者主要用于清热泻火、除烦止渴,临床用量范围较大.王庆国教授临证50 余载,临床应用生石膏颇多.王教授认为石膏性凉,最多谓微寒,临床可放胆用之,不必太过拘谨.生石膏在发挥清热泄火、除烦止渴功效时,还有辛而能散能行,甘而能缓能润之性,凉而无大寒郁闭之畏的优点,是清阳明气热之专药,亦可广泛应用于表里内外三焦实邪热盛.小剂量(10～30 g)石膏清郁热微热,适用于里热兼表、表邪仍在者,肺胃郁热、热势轻浅者和本虚内热者.中剂量(30～50 g)石膏清脏腑里热,适用于郁热壅肺者、胃热诸证者和其他脏腑里热者.大剂量(50～100 g)石膏清血热壮热,适用于伤寒阳明热盛、或温病气分热炽者,血分热盛的斑疹出血,湿热偏盛的风湿痹症.并结合临床验案说明王教授临床应用生石膏的经验.
Abstract
Shigao(Gypsum Fibrosum)boasts extensive applications in managing various diseases,with its unprocessed form primarily employed for its heat-clearing and fire-purging properties,as well as its capacity to dispel vexation and alleviate thirst.Professor Wang Qingguo brings over 50 years of clinical experience to his practice,where unprocessed Shigao has played a prominent role in numerous cases.Professor Wang advocates for a bold utilization of the medicinal ingredient in clinical settings,emphasizing its cool or,at most,slightly cold nature.Beside the aforementioned functions,unprocessed Shigao is noted for its pungency,which allows for dispersion and mobility,as well as its sweetness,offering a capacity to slow and moisten without the risk of severe cold oppression.Consequently,it emerges as a distinctive remedy for addressing yangming qi heat,finding broad applications in treating internal and external heat,spanning all three jiao areas.In clinical practice,a modest dosage(10-30 g)of unprocessed Shigao demonstrates effectiveness in alleviating both internal and external heat.It proves beneficial for individuals with external syndromes or those contending with suppressed heat in the lungs and stomach.This dosage is particularly advantageous for cases involving mild heat conditions and internal heat within a deficiency context.With a moderate dosage(30-50 g),the focus shifts to clearing internal heat within the organs.This is particularly suitable for individuals experiencing heat in the lungs and stomach,and internal heat in other organs.A substantial dosage(50-100 g)targets heat in the blood,making it suitable for conditions such as typhoid fever,yangming heat,or warm diseases characterized by intense heat in the qi level of the body.Additionally,it proves effective in cases of bleeding rashes with heat in the blood region and rheumatism bi syndromes with a prevalence of dampness-heat.To showcase Professor Wang's clinical acumen in the application of unprocessed Shigao,this article incorporates pertinent clinical cases.
