摘要
随着冠心病支架植入术的普及,术后伴失眠的患者越来越多见.本文总结了刘春甫教授对冠心病支架植入术后伴失眠的病因病机认识和临床经验.刘教授认为本病主要责之于心、肝、肾三脏.病在心者,安神为主;病在肝者,镇魂为主;病在肾者,定志为主.善用大剂量酸枣仁以宁心安神;用中药汤剂送服琥珀粉以镇心安神、平肝镇魂;用大量煅磁石、煅石决明、龙骨、牡蛎、龙齿等重镇之品安定神魂之乱.同时提出瘀血贯穿疾病全过程,故在治疗上皆佐丹参饮以活血化瘀.本文亦附验案1 则以供同道探讨.
Abstract
With the widespread adoption of coronary stent implantation,the occurrence of postoperative insomnia has become increasingly prevalent.This article provides a comprehensive overview of Professor Liu Chunfu's insights into the etiology,pathogenesis,and clinical experience in treating insomnia following coronary stent implantation.Professor Liu posits that the primary organs involved in this condition are the heart,liver and kidney.For cases affecting the heart,the focus is on calming the spirit;for liver-related issues,emphasis is on soul-soothing;and for kidney-related problems,stabilizing the will takes precedence.Professor Liu advocates the strategic use of high-dose Suanzaoren(Semen Ziziphi Spinosae)to tranquilize the mind.Traditional Chinese medicine decoctions,incorporating Hupo(Succinum)powder,are employed for calming the mind and soothing liver and soul.Additionally,a combination of high doses of Duancishi(Magnetitum Praeparatum),Duanshijueming(Concha Haliotidis Praeparatum),Longgu(Os Draconis),Muli(Concha Ostreae),Longchi(Dens Draconis),among others,is used to stabilize the mind and soothe the spirit with potent ingredients.Professor Liu also highlights the role of blood stasis throughout the disease process,advocating the use of Danshen Beverage(Salvia Beverage)to promote blood circulation and eliminate stasis.A case study is presented for further discussion among peers.
