摘要
发声性抽动是抽动障碍的一种表现形式,以咽部发声、不自主重复及模仿他人语言甚至秽语为主要表现.王素梅教授认为发声性抽动病位在咽喉,位处上焦,多条经脉所过;主要病机为痰饮内停、气失宣降、郁而化火,治疗采用上焦宣痹汤,重在清轻宣散、畅达气机,以发散郁热和黏滞之痰.并结合临床病案阐述王教授运用上焦宣痹汤治疗儿童发声性抽动的经验.
Abstract
Vocal tic disorder is a manifestation of tic disorders,characterized by pharyngeal vocalization,involuntary repetition or imitation of others'speech,and even coprolalia.Professor Wang Sumei believes that the pathological site of vocal tic disorder is in the throat,located in the upper focus,traversed by multiple meridians.The main pathogenesis involves congestion of phlegm-rheum,obstructed dispersing and descending of qi,and stagnation turning into fire.Treatment involves the use of Shangjiao Xuanbi Decoction(Upper Jiao Painful Obstruction Resolving Decoction),focusing on dispersing stagnation and clearing heat,thereby dispersing phlegm.This paper,supplemented by clinical case records,elucidates Professor Wang Sumei's treatment experience and medication principles.
