摘要
心房颤动(atrial fibrillation,AF)是临床中最常见的心律失常,射频消融术(radiofrequency cath-eter ablation,RFCA)后患者出现房颤、房扑、房速发作,可归属于中医"心悸""惊悸""怔忡"范畴.络风内动是RFCA术后复发的核心病机,包括心神失养、络虚生风,火热生风、耗心气阴及外风引动内风三方面.息风通络是RFCA术后复发的重要治法,具体包括补络养心、祛瘀宁风,清热息风、益气养阴和搜风通络、攘外安内.并结合临床案例对基于络风内动学说辨治RFCA术后复发进行阐述.
Abstract
Atrial fibrillation(AF)is the most common arrhythmia in clinical practice.The recurrence of atrial fibrillation,atrial flutter,and atrial tachycardia in patients after radiofrequency catheter ablation(RFCA)can be categorized under the TCM diagnoses of xinji(palpitations),jingji(palpitations due to fright),and zhengzhong(severe palpitations).The core pathogenesis of RFCA postoperative recurrence is the internal stirring of collateral vessels wind,which includes three main aspects:the deficiency of heart-spirit leading to internal wind,the generation of wind from fire-heat and depletion of heart qi and yin,and the triggering of internal wind by external wind.The key treatment strategies for postoperative recurrence of RFCA involve calming internal wind and tonifying the collaterals,which include nourishing and tonifying the heart,eliminating stasis and calming wind,clearing heat and calming wind,boosting qi and nourishing yin,and dispelling external wind to pacify internal wind.This paper discusses these treatment principles through a clinical case study.
