Using a case study of invited academician Tong Xiaolin during teaching ward rounds for a patient with metabolic syndrome complicated by coronary heart disease in the Department of Endocrinology of Guang'anmen Hospital as an example, this article elaborates on the traditional Chinese medicine diagnostic and treatment strategies, the progression of the diseases, and key points in diagnosis and treatment of metabolic syndrome combined with coronary heart disease. It is believed that emphasis should be placed on distinguishing the similarities and differences between Pidan and Xiaodan, and grasping the distinct trends of disease development.Pidan is prone to comorbidities with large blood vessels and microvascular complications, while Xiaodan is associated more with microvascular complications. In this paper the approaches highlighte the importance of combined treatment of both meridians and collateral vessels, and following the theory of liver treatment.Additionally,it is emphasized to pay attention to the use of the state-target treatment based on syndrome defferentiation system to reduce the use of Western medicine.
