摘要
目的 探讨三乌胶丸对中风后遗症患者临床疗效及神经功能恢复的影响.方法 选择 2020 年 3 月-2022 年 9 月于中南大学湘雅医院住院治疗的中风后遗症患者 106 例,随机分为西药组与联合中药组,各 53 例.西药组使用阿司匹林治疗,联合中药组使用阿司匹林联合三乌胶丸治疗.统计2 组临床疗效、中医证候积分、血清细胞因子水平、血液流变学指标、神经功能及生活质量变化.结果 治疗 4 周后,联合中药组总有效率(90.57%,48/53)高于西药组(75.47%,40/53)(P＜0.05).与治疗前比较,治疗4周后2组中医证候积分各项得分、血清S100-β水平、红细胞积压(HCT)、血浆比黏度(PV)、全血低切黏度(WBLSV)、全血高切黏度(WBHSV)及美国国立卫生研究院卒中量表(NIHSS)评分、神经功能缺损(NDS)评分降低,联合中药组低于西药组(P＜0.05);治疗4周后2组血清脑源性神经营养因子(BDNF)、内皮衍生超极化因子(EDHF)、一氧化氮(NO)水平及Berg平衡量表评分、Fugl-Meyer运动功能量表(FMA)下肢评分及生活质量各项评分均升高,联合中药组高于西药组(P＜0.05).结论 三乌胶丸可缓解中风后遗症患者的中医证候,改善血清细胞因子水平及血液流变学,促进患者神经功能、下肢功能恢复,提高患者生活质量,具有较好的治疗效果.
Abstract
Objective To explore the effect of Sanwu capsules on clinical efficacy and neurological functional recovery in patients with stroke sequelae.Methods 106 patients with stroke sequelae admitted to Xiangya Hospital of Central South University from March 2020 to September 2022 were randomly divided into the Western medicine group(53 cases)and the combined Chinese medicine group(53 cases).The Western medicine group was treated with aspirin,and the combined Chinese medicine group was treated with aspirin plus Sanwu capsules.Clinical efficacy,the scores of traditional Chinese medicine syndrome,levels of serum cytokines,hemorheology indexes,neurological function and quality of life of two groups were statistically analyzed.Results After 4 weeks of treatment,the total effective rate of the combined Chinese medicine group(90.57%,48/53)was higher than that of the Western medicine group(75.47%,40/53)(P<0.05).Compared with before treatment,after 4 weeks of treatment,the scores of traditional Chinese medicine syndrome,serum level of S100-β,hematocrit(HCT),plasma viscosity(PV),whole blood low shear viscosity(WBLSV),whole blood high shear viscosity(WBHSV),scores of National Institutes of Health Stroke Scale(NIHSS)and neurological deficits(NDS)decreased,and the combined Chinese medicine group was lower than the Western medicine group(P<0.05);The levels of serum brain-derived neurotrophic factor(BDNF),endothelium-derived hyperpolarization factor(EDHF)and nitric oxide(NO),and scores of Berg balance scale,lower-extremity Fugl-Meyer Assessment(FMA)and quality of life were all elevated in both groups,and the combined Chinese medicine group was higher than the Western medicine group(P<0.05).Conclusion Sanwu capsules can relieve the traditional Chinese medicine syndrome of stroke sequelae patients,improve the levels of serum cytokines and hemorheology,promote the recovery of nerve function and lower limb function,improve the quality of life of patients,and has the good therapeutic effect.
