Effect of scalp clustering acupuncture combined with knee flexion standing at different angles on displacement of center of gravity and gait of patients with knee hyperextension after stroke
Objective To observe the effect of scalp clustering acupuncture combined with knee flexion standing at different angles on displacement of center of gravity and gait of patients with knee hyperextension after stroke. Methods 80 patients with knee hyperextension after stroke admitted to Department of Rehabilitation Medicine of Sanya Hospital of Traditional Chinese Medicine from September 2021 to January 2023 were divided into the observation group and the control group according to random number table method, with 40 cases in each group. The control group was given routine rehabilitation treatment, while the observation group was given scalp clustering acupuncture and long needle retaining method on the basis of the treatment of the control group.During needle rataining period, the patients were trained to stand with knees bent at different angles for 8 weeks. Before treatment, after 4 and 8 weeks of treatment, the maximum displacement of center of gravity towards the affected side, healthy side, front side and back side,as well as gait speed, gait cycle and deviation in foot step length between healthy and affected sides were compared between two groups when the patients stood on two legs quietly without assistance. Results Compared with before treatment, after 4 and 8 weeks of treatment, the maximum displacement of center of gravity towards the affected and healthy sides in two groups when the patients stood on two legs quietly was shortened; And after 8 weeks of treatment, the maximum displacement of center of gravity towards the affected and healthy sides in the observation group when the patients stood on two legs quietly and the maximum displacement of center of gravity towards the affected side in the control group were shorter than those after 4 weeks of treatment(P<0.05).After 4 weeks of treatment, the maximum displacement of center of gravity towards the affected side in the observation group was shorter than that in the control group. After 8 weeks of treatment, the maximum displacement of center of gravity towards the affected and healthy sides in the observation group was shorter than that in the control group(P<0.05).Compared with before treatment, after 4 and 8 weeks of treatment, the maximum displacement of center of gravity towards the front and back sides when the patients stood on two legs quietly in two groups was shortened, and after 8 weeks of treatment it was shorter than that after 4 weeks of treatment(P<0.05).After 4 weeks of treatment, the maximum displacement of center of gravity towards the front side in the observation group was shorter than that in the control group; And after 8 weeks of treatment, the maximum displacement of center of gravity towards the front and back sides in the observation group was shorter than that in the control group(P<0.05).Compared with before treatment, after 4 and 8 weeks of treatment, the gait speed of two groups was accelerated; And after 8 weeks of treatment,it was faster than that after 4 weeks of treatment, and the gait cycle and deviation of foot step length in two groups were shortened; And after 8 weeks of treatment, they were shorter than those after 4 weeks of treatment(P<0.05).After 4 or 8 weeks of treatment, the gait cycle and deviation of foot step length in the observation group were shorter than those in the control group(P<0.05).Conclusion Scalp clustering acupuncture combined with knee flexion standing training at different angles on the affected side can improve the standing stability of patients with knee hyperextension after stroke and improve their walking ability.
