Teaching ward round transcript on type 2 diabetic peripheral neuropathy complicated with insulin resistance case by academician Tong Xiaolin
Using a case study of invited academician Tong Xiaolin during teaching ward rounds for a patient with type 2 diabetic peripheral neuropathy complicated with insulin resistance in the Department of Endocrinology of Guang'anmen Hospital as an example,this article elaborates on the traditional Chinese medicine diagnostic and treatment strategies,the progression of the diseases,and key points in diagnosis and treatment of type 2 diabetic peripheral neuropathy combined with insulin resistance.It is believed that emphasis should be placed on distinguishing the similarities and differences between lean diabetes mellitus and obese diabetes mellitus,and grasping the distinct trends of disease development.Although diabetes mellitusis type 2 is usually combined with insulin resistance,the failure rate of islet function in lean diabetes mellitus type 2 patients is accelerated,and even in the early stage of disease,islet function can be damaged.Therefore,the core pathogenesis should be spleen deficiency.In the treatment,attention should be paid to invigorating spleen and replenishing Qi according to the theory of spleen treatment,and using the state-target treatment based on syndrome defferentiation system to improve clinical efficacy.
