Comparison of clinical effects and complications between laparoscopic and open hysterectomy in patients with adenomyosis
Objective To compare the clinical effects and complications of laparoscopic and open hysterectomy in patients with adenomyosis(AM).Methods 89 patients with AM were selected and divided into the laparoscopy group(46 cases)and the laparotomy group(43 cases)according to different surgical methods.Perioperative indexes,visual analogue scale(VAS),stress response related indexes[cortisol(Cor),serum angiotensin Ⅱ(Ang Ⅱ)],inflammatory factor levels[C-reactive protein(CRP),interleukin-6(IL-6)],ovarian function and postoperative complications were compared between two groups.Results There was no statistically significant difference in operation time between two groups(P<0.05).The amount of intraoperative bleeding in the laparoscopy group was less than that in the laparotomy group(P<0.001),and the length of hospitalization in the laparoscopy group was shorter than that in the laparotomy group(P<0.001).The VAS scores at 6,12 and 24 hours after operation in the laparoscopy group were lower than those in the laparotomy group(P<0.05).The levels of Cor and Ang Ⅱ in the laparoscopy group were lower than those in the laparotomy group(P<0.001).The levels of CRP and IL-6 in the laparoscopy group were lower than those in the laparotomy group(P<0.001).The E2 level in the laparoscopy group was higher than that in the laparotomy group(P<0.001),while the LH and FSH levels in the laparoscopy group were lower than those in the laparotomy group(P<0.001).The incidence of postoperative complications in the laparoscopy group(6.52%)was lower than that in the laparotomy group(20.93%)(P=0.047).Conclusion Compared with open hysterectomy,laparoscopic hysterectomy is more effective in the treatment of AM.
