Observation on the therapeutic effect of Qishen Yiqi Dropping Pills on reperfusion injury in patients with acute myocardial infarction of blood stasis due to qi deficiency type after percutaneous cornary intervention surgery
Objective To explore the protective effect of Qishen Yiqi Dropping Pill on reperfusion injury of myocardial microvasculature after percutaneous coronary intervention (PCI) for acute myocardial infarction of blood stasis due to qi deficiency type. Methods 92 patients with acute myocardial infarction of blood stasis due to qi deficiency type admitted to Baoshan Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2017 to June 2019 were randomly divided into the observation group and the control group,with 46 cases in each group. Both groups were treated with PCI. After surgery,the control group received oral aspirin enteric-coated tablets and clopidogrel hydrogen sulfate tablets,while the observation group received additionally Qishen Yiqi Dropping Pills on the basis of the treatment of the control group. Both groups were treated for 4 weeks. The traditonal Chinese medicine (TCM) symptom scores and therapeutic effects before and after treatment were compared. The levels of superoxide dismutase (SOD),L-glutathione reduced (GSH),creatine kinase isoenzymes (CK-MB),myoglobin (Mb) and cardiac troponin cardiac troponin I (cTnI) were measured by ELISA kits. And left ventricular ejection fraction (LVEF),left ventricular end diastolic diameter (LVEDD) and left ventricular end systolic diameter (LVESD) were monitored by ultrasonic detector. The incidence of adverse reactions during treatment was compared between two groups. Results The total effective rate of the observation group was 86.96%,which was significantly higher than that of the control group (60.87%) (P<0.05). After treatment,chest pain,chest tightness,palpitation,shortness of breath,spiritlessness and fatigue,cough and phlegm accumulation,numbness of hands and feet,and dry mouth without desire to drink in the observation group were significantly lower than those in the control group (P<0.01 or P<0.05). After 4 weeks of treatment,the serum levels of SOD and GSH in the observation group were significantly higher than those in the control group (P<0.01). CK-MB,cTnI and Mb levels in the observation group were significantly lower than those in control group (P<0.01). After 4 weeks of treatment,LVESD in the observation group (26.65±2.69)mm was lower than that in the control group (30.54±3.33)mm (P<0.01),LVEDD in the the observation group (41.36±3.64)mm was lower than that in the control group (45.73±3.41)mm (P<0.01),and LVEF in the observation group (66.54±5.04)% was higher than that in the control group (61.43±6.26)% (P<0.01). Conclusion Qishen Yiqi Dropping Pill has a definite therapeutic effect on patients with reperfusion injury after interventional treatment of acute myocardial infarction of blood stasis due to qi deficiency type. It can reduce oxygen free radicals in the patient's body,improve heart function and traditional Chinese medicine symptoms,reduce CK-MB,cTnI,Mb activities,increase SOD and GSH activities,and have a significant protective effect on myocardial microvasculature after myocardial ischemia-reperfusion injury (MIRI).
