摘要
目的 探讨益气通腑法中药灌肠辅助治疗慢性阻塞性肺疾病合并呼吸衰竭疗效.方法 按照随机数表法将82例慢性阻塞性肺疾病伴呼吸衰竭患者分成对照组与观察组,各41例.对照组在西医常规治疗基础上予无创正压通气等西医治疗,观察组在对照组基础上联合益气通腑法中药灌肠治疗.观察2组治疗前后肺功能指标、血清炎性指标肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)、白细胞介素8(IL-8),血气分析指标动脉血氧分压(PaO2)、血氧饱和度(SaO2)、二氧化碳分压(PaCO2),慢阻肺病评估量表(CAT)评分,mMRC评分,慢性阻塞性肺疾病生存状况评估量表(BODE)评分变化,并比较2组治疗总有效率和不良反应总发生率.结果 观察组治疗后各项肺功能指标、PaO2、SaO2指标及治疗总有效率均高于对照组(P＜0.05),TNF-α、CRP、IL-8、PaCO2、CAT评分、mMRC评分、BODE评分及不良反应总发生率均低于对照组(P＜0.05).结论 益气通腑法中药灌肠能够提高慢性阻塞性肺疾病伴呼吸衰竭患者肺通气功能,调节血气指标,抑制体内炎症反应,改善临床预后,且不良反应较少.
Abstract
Objective To explore the therapeutic effect of traditional Chinese medicine enema with Yiqi Tongfu therapy on patients with chronic obstructive pulmonary disease(COPD)complicated with respiratory failure.Methods 82 COPD patients with respiratory failure were divided into the control group and the observation group according to random number table method,with 41 cases in each group.The control group was treated with non-invasive positive pressure ventilation and other Western medicine treatment on the basis of conventional Western medicine treatment,and the observation group was additionally treated with traditional Chinese medicine enema with Yiqi Tongfu therapy on the basis of the treatment of the control group.The changes of the lung function indicators,serum inflammatory indicators[tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-8(IL-8)],blood gas analysis indicators[arterial partial pressure of oxygen(PaO2),oxygen saturation of blood(SaO2),partial pressure of carbon dioxide(PaCO2)],COPD Assessment Test(CAT)score,mMRC score,and the Body-Mass Index,Airflow Obstruction,Dyspnea,and Exercise Capacity Index(BODE)score before and after treatment in two groups were observed,and the total effective rate of treatment and incidence of adverse reactions between two groups were compared.Results After treatment,the lung function indicators,the levels of PaO2 and SaO2,and total effective rate in the observation group were higher than those in the control group(P＜0.05),and the levels of TNF-α,CRP,IL-8 and PaCO2,and the CAT score,mMRC score and BODE score,and the total incidence of adverse reactions were lower than those in the control group(P＜0.05).Conclusion The traditional Chinese medicine enema with Yiqi Tongfu therapy can improve lung ventilation function of COPD with respiratory failure,regulate blood gas analysis indicators,inhibit inflammatory reactions in the body,improve clinical prognosis of patients,and have fewer adverse reactions.
