自拟黄精降压汤治疗H型高血压亚组分析

摘要
目的 观察自拟黄精降压汤治疗气虚型高血压伴高同型半胱氨酸血症(HHcy)亚组疗效分析.方法 从自拟黄精降压汤治疗气虚型高血压临床疗效观察项目中选取同型胱氨酸(Hcy)≥10 μmol·L-1的亚组人群65例患者进行确证性亚组分析.对照组给予口服苯磺酸氨氯地平片每次5 mg,每日1次:观察组在对照组用药基础上给予口服自拟黄精降压汤治疗,每日2次,2组均治疗8周.比较2组治疗前后中医症候积分、血压、同型半胱氨酸、血脂变化.结果 治疗后,观察组中医症候积分、总有效率高于对照组(P＜0.05);治疗后观察组诊室血压、家庭血压、24 h动态血压SBP值、同型半胱氨酸均明显低于对照组(P＜0.05):治疗后观察组低密度脂蛋白胆固醇低于对照组,但差异无统计学意义(P＞0.05).结论 自拟黄精降压汤可明显改善气虚型H型高血压患者临床症状,降低患者血浆同型半胱氨酸值,安全有效.
Abstract
Objective To observe the therapeutic effect of self-formulated solomonseal rhizome decoction for dropping blood pressure on Qi-deficiency type hypertension with hyperhomocysteinemia subgroup.Methods A confirmatory subgroup analysis was conducted on 65 patients with homocystine(Hcy)≥ 10μmol·L-1 selected from the clinical efficacy observation project of self-formulated solomonseal rhizome decoction for dropping blood pressure in the treatment of Qi-deficiency type hypertension.The control group received treatment with oral amlodipine besylate tablets,5mg/time,once a day,and the observation group received oral self-formulated solomonseal rhizome decoction for dropping blood pressure treatment on the basis of the treatment of the control group,twice a day.Both groups were treated for 8 weeks.The changes in traditional Chinese medicine(TCM)syndrome scores,blood pressure,homocysteine,and blood lipids before and after treatment in both groups were observed and compared.Results After treatment,the differences in TCM syndrome scores and the effective rate in the observation group were significantly higher than those in the control group(P＜0.001).After treatment,the clinic blood pressure,home blood pressure,24-hour ambulatory blood pressure,and homocysteine in the observation group were significantly lower than those in the control group(P＜0.05).After treatment,the level of low-density lipoprotein cholesterol in the observation group was lower than that in the control group,but there was no statistically significant difference(P＞0.05).Conclusion Self-formulated solomonseal rhizome decoction for dropping blood pressure can significantly improve the clinical symptoms and reduce blood homocysteine levels in patients with Qi-deficiency type H-type hypertension.This therapy is safe and effective.
