Relationship between inflammatory mediators and pain,oral health,and quality of life in patients with pulpitis
Objective To study the changes of the levels of inflammatory mediators in patients with pulpitis and their relationship with postoperative pain,oral health and quality of life after root canal treatment.Methods A total of 104 patients with pulpitis admitted to Nanjing Jiangning Hospital from January 2021 to December 2022 were selected as the observation group.All patients underwent root canal treatment and were observed for 1 month after surgery.112 healthy subjects who received orthodontic tooth extraction treatment in the same hospital during the same period were selected as the control group.The levels of inflammatory mediators in serum and gingival crevicular fluid of two groups were compared.The scores of pain,oral health and quality of life before and after treatment in the observation group were compared.The relationship between the changes of inflammatory mediators levels in patients with pulpitis and the scores of postoperative pain,oral health and quality of life after root canal treatment was analyzed.Results Before treatment and after 1 month of treatment,the levels of matrix metalloproteinase-8(MMP-8),interleukin(IL)-8,IL-6 and tumor necrosis factor-α(TNF-α)in serum and gingival crevicular fluid in the observation group were higher than those in the control group before treatment(P<0.05).After 1 month of treatment,the levels of MMP-8,IL-8,IL-6 and TNF-α in serum and gingival crevicular fluid in the observation group were lower than those before treatment(P<0.05).After 1 month of treatment,compared with before treatment,the scores of pain and oral health of patients with pulpitis decreased,while compared with before treatment,the scores of social function,masticatory function,material life and psychological function increased(P<0.05).Pearson correlation analysis results showed that the changes of MMP-8,IL-8,IL-6 and TNF-α levels in serum and gingival crevicular fluid of patients with pulpitis were significantly positively correlated with postoperative pain and oral health scores after root canal treatment,and significantly negatively correlated with the scores of social function,masticatory function,material life and psychological function.Conclusion The occurrence of pulpitis can cause the increase in the levels of inflammatory mediators in serum and gingival crevicular fluid of the patients.These changes in levels of inflammatory mediators are significantly correlated with postoperative pain,oral health and quality of life of patients after root canal treatment.
