Exploration of the prediction model for prosthesis models in patients undergoing total knee arthroplasty
Objective To explore the construction of prediction models for the size of femur(F)and tibia(T)implants in total knee arthroplasty(TKA),in order to provide reference for optimizing the matching of surgical instruments and achieve the goal of reducing costs and improving efficiency.Methods A retrospective study was conducted to collect the data of 440 patients who underwent TKA in Guangdong Provincial Hospital of Chinese Medicine from January 2019 to December 2019.The collected data included the patient's gender,age,height,body mass,brand of prosthesis,anterior posterior(AP)diameter and medial lateral(ML)diameter of the femoral and tibial prostheses.t test,Mann-Whitney U test,and χ2 test were used to compare between different groups,and multiple regression analysis was used in the prediction model.Results The mean AP diameter of femur of 440 patients was(58.6±3)mm,and the mean ML diameter of tibia was(68.8±3.7)mm.AP diameter of the most commonly used femoral prosthesis was 58mm(22.27%),and AP diameter of the commonly used femoral prosthesis ranged from 55mm-62mm(86.14%).ML diameter of the most commonly used tibial prosthesis was 68mm(33.86%),and ML diameter of the commonly used tibial prosthesis ranged from 63mm-74mm(93.86%).The prediction models for size of total knee prosthesis were constructed,including AP diameter of femur(mm)=29.1 22+18.759×height(m)+0.039×body mass(kg)-1.397×sex(male=1,female=2),ML diameter of tibia(mm)=37.880+20.675×height(m)+0.036×body mass(kg)-2.134×sex(male=1,female=2),and prediction models of AP diameter of femur and ML diameter of tibia by gender.The research team applied the information above to optimize the configuration of surgical instruments for TKA and the reserve models of implanted prostheses.Conclusion The prediction model constructed by using the indicators of gender,height and weight of patients can help perioperative professionals to predict the model size of the implanted prosthesis in TKA and matching surgical tools before surgery without increasing economic costs and material consumption,in order to optimize the matching and inventory of surgical instruments and improve the safety and efficiency of surgery.
