Relationship between thyroid cancer risk and plasma 25 hydroxyvitamin D and vitamin D binding proteins
Objective To analyze the relationship between the risk of thyroid cancer and plasma levels of 25 hydroxyvitamin D[25(OH)D]and vitamin D binding protein(VDBP).Methods 102 patients with thyroid cancer diagnosed and treated in the Second Affiliated Hospital of Xi'an Jiaotong University from January 2022 to September 2023 were selected as the malignant group,and 102 patients with benign thyroid nodules diagnosed and treated during the same period were selected as the benign group.The risk factors for thyroid cancer and the diagnostic value of plasma 25(OH)D and VDBP for thyroid cancer were analyzed.Results Compared with the benign group,the levels of plasma 25(OH)D and VDBP were lower in the malignant group(P<0.05),the serum free thyroxine 4(FT4)level and urinary iodine/creatinine value were higher in the malignant group(P<0.05),and there was no statistically significant difference in other factors in the malignant group(P>0.05).Using group as the dependent variable,and factors with statistically significant differences in univariate analysis as the independent variable,binary Logistic regression analysis showed that the levels of high 25(OH)D and high VDBP were protective factors for thyroid cancer(P<0.05),while high FT4 level and high urinary iodine/creatinine value were risk factors for thyroid cancer(P<0.05).Rreceiver operating characteristic curve(ROC curve)analysis showed that area under the curve(AUC)of plasma 25(OH)D,VDBP,and their combination for diagnosing thyroid cancer was 0.840,0.811,and 0.862,respectively,the sensitivity was 91.2%,93.1%,and 87.3%respectively,and the specificity was 66.7%,60.8%,and 86.3%,respectively.Conclusion The risk of thyroid cancer is related to the decrease in plasma 25(OH)D and VDBP levels,as well as the increase in serum FT4 level and urine iodine/creatinine value.The combination of plasma 25(OH)D and VDBP has high diagnostic value for thyroid cancer.
