Risk factors and preventive measures for postoperative infection in patients with hepatolithiasis
Objective To explore risk factors and preventive measures for postoperative infection in patients with hepatolithiasis.Methods 118 patients with hepatolithiasis who underwent partial hepatectomy in the First Affiliated Hospital of Xinjiang Medical University from January 2022 to December 2023 were retrospectively enrolled and divided into the infection group and the non infection group,with 59 cases in each group.The infection status and distribution of pathogens in patients were detected and analyzed.Logistic regression analysis was used to analyze the influencing factors of postoperative complications in patients with hepatolithiasis.The nomogram model was constructed.Results Among 59 postoperative infectious patients,the infection sites included incisions,abdominal cavity,urinary tract,lungs,and diaphragm,with incisions accounting for the highest proportion at 42.37%.The proportion of patients with age≥60 years,diabetes,choledochojejunostomy,preoperative albumin<35 g·L-1,intraoperative blood loss≥400 mL,and operation time≥300 min in the infection group was higher than that in the non infection group(P<0.05).Age≥60 years,diabetes,choledochojejunostomy,preoperative albumin<35 g·L-1,intraoperative blood loss≥400 mL,and operation time≥300 min were risk factors for infectious complications after partial hepatectomy in patients with hepatolithiasis(P<0.05).The nomogram model had a high consistency between the actual probability and the predicted probability,which could effectively predict the risk of complications.Conclusion The risk factors of infectious complications after partial hepatectomy in patients with hepatolithiasis include age≥60 years,diabetes mellitus,choledochojejunostomy,preoperative albumin<35 g·L-1,intraoperative blood loss≥400 mL,and operation time≥300 min.Targeted preventive measures should be taken for patients with combined risk factors to reduce the incidence of postoperative infections.
