Distribution characteristics and significance of human papillomavirus genotype in elderly female patients
Objective To analyze the distribution characteristics of human papillomavirus(HPV)genotype in elderly female patients,and provide theoretical basis for detection,prevention and HPV vaccination of cervical cancer in elderly women.Methods 7716 female patients who visited Jilin Cancer Hospital from July 2019 to February 2023 were collected for HPV testing.Among them,6722 female patients under the age of 60 were selected as the control group,and 994 female patients aged 60 and above were selected as the elderly group.The detection results of HPV using flow-through hybridization method were analysed retrospectively.The detection of HPV infection and the distribution characteristics of infection in different age groups,as well as the proportion of high-risk and low-risk subtypes in HPV positive patients,were observed.Results The total positive rate of elderly patients was 30.28%,with a single infection positive rate of 22.23%and a multiple infection positive rate of 8.05%.The total positive rate of the control group was 22.99%,with a single infection positive rate of 18.68%and a multiple infection positive rate of 4.31%.There was statistically significant difference(P<0.01).The age group with the lowest HPV infection positive rate was 75-79 years old,with a positive rate of 12.50%,while the age group with the highest positive rate was 60-64 years old,with a positive rate of 33.79%.Single infection was predominant in all age groups,with the lowest HPV positive rate of 10%among individuals aged 75-79 years old,and the highest HPV positive rate of 24.56%among individuals aged 60-64 years.The detection rate of low-risk HPV81(CP8304)type was the highest in the control group and the elderly group,and the detection rate of HPV11 type was the lowest in both groups.The top five high-risk HPV types in the elderly group and the control group were HPV16,HPV52,HPV58,HPV53,and HPV51,all of which were predominantly monoinfections.The highest infection rate among all age groups in the elderly group was HPV16.Conclusion Among elderly female patients,the HPV positive rate is higher than that of young female patients,and it is mainly caused by single and high-risk infections.The HPV infection positive rate is highest in the age group of 60-64 years old.The top five high-risk HPV types with the highest detection rates are HPV16,HPV52,HPV58,HPV53,and HPV51.
