心理韧性在老年2型糖尿病患者低血糖恐惧与心理痛苦间的中介效应

目的:探索心理韧性在老年2型糖尿病患者低血糖恐惧与心理痛苦间的中介效应.方法:2023年3-6月,采用一般资料调查问卷、低血糖恐惧量表、心理韧性量表、心理痛苦量表对成都市4个社区共230例老年2型糖尿病患者进行调查.结果:老年2型糖尿病患者低血糖恐惧总分为(26.70±6.20)分,心 理韧性总分为(60.62±4.39)分,心理痛苦总分为(34.21±6.28)分.低血糖恐惧与心理韧性呈负相关(r=-0.565,P＜0.01),与心 理痛苦呈正相关(r=0.712,P＜0.01).低血糖恐惧可直接影响老年2型糖尿病患者的心理痛苦,还能通过心理韧性的中介作用间接影响心理痛苦,中介效应占总效应的18.7％.结论:老年2型糖尿病患者低血糖恐惧和心理痛苦感较强,心理韧性水平有待提升,心理韧性在低血糖恐惧与心理痛苦间起着部分中介作用.社区医务人员应采取针对性措施提高老年2型糖尿病患者心理韧性水平,降低其低血糖恐惧与心理痛苦.
Mediation effect of psychological resilience between hypoglycemic fear and psychological pain in elderly patients with type 2 diabetes
Objective:To explore the mediation effect of psychological resilience between hypoglycemic fear and psychological pain in elderly patients with type 2 diabetes.Methods:From March to June 2023,230 elderly patients with type 2 diabetes from 4 communities in Chengdu were investigated using general information questionnaire,hypoglycemia fear scale,psychological resilience scale and psychological pain scale.Results:The total score of hypoglycemic fear in elderly patients with type 2 diabetes was(26.70±6.20)points,the total score of psychological resilience was(60.62±4.39)points,and hypoglycemic fear was(34.21±6.28)points.Hypoglycemic fear was negatively correlated with psychological resilience(r=-0.565,P＜0.01),and positively correlated with psychological pain(r=0.712,P＜0.01).Hypoglycemic fear could directly affect the psychological pain of elderly patients with type 2 diabetes,and can also indirectly affect the psychological pain through the mediation of psychological resilience.The mediation effect accounted for 18.7％of the total effect.Conclusion:The elderly patients with type 2 diabetes have relatively strong hypoglycemic fear and psychological pain,and the level of psychological resilience needs to be improved.Psychological resilience plays a partial mediating role between hypoglycemic fear and psychological pain.Community healthcare workers should take targeted measures to improve the psychological resilience level of these patients and reduce their hypoglycemic fear and psychological pain.
