Curative effect of external application of traditional Chinese medicine hot pack in the treat-ment of acute ankle sprain caused by naval military training
Objective To assess the curative effect of external application of traditional Chinese medicine hot pack in the treatment of acute ankle sprain(AAS)caused by naval military training.Methods A total of 120 patients with AAS were prospectively selected from our department from Oct.2020 to Jan.2023,including 103 males and 17 females aged 18-38(mean 26.1)years,with the average course of disease being(8.3±1.9)h.The AAS was caused by run training in 67 patients and weight training in 53 patients.According to the random number table method,patients were divided into control group(n=60,symptomatic treatment combined with external applica-tion of diclofenac diethylamine soft emulsion)and observation group(n=60,symptomatic treatment combined with external application of traditional Chinese medicine hot pack,with the formula being 6 g herba speranskiae tubercula-tae,12 g pseudocydonia sinensis,4 g kusnezf monkshood root,8 g notopterygium root,12 g marsdenia offcinalis tsi-ang,10 g ramuli mori,6 g boswellia carteri,8 g angelica,6 g aconitum carmichaelii debeaux,5 g myrrh,8 g davallia trichomanoides blume,6 g cinnamo,10 g Chinese angelica,and 1 big jujube).The clinical effective rate,swelling score,visual analogue scale/score(VAS),American Orthopaedic Foot and Ankle Society(AOFAS)ankle-hindfoot score,thickness of the anterior talofibular ligament(ATFL)and calcaneofibular ligament(CFL),ankle joint circ-umference,ankle plantar flexion/dorsiflexion range of motion(ROM)and quality of life were compared between the two groups.Results After two weeks of treatment,the clinical effective rate was 90.0%in the experimental group,much higher than the 68.3%in the control group(P<0.05).The AOFAS ankle-hindfoot score was 89.7±7.5 in the experimental group,much higher than that in the control group(72.8±6.1),while the swelling score and VAS were much lower(3.9±0.5 vs.6.7±0.6 and 2.2±0.3 vs.3.1±0.4,both P<0.05).Moreover,the experimental group revealed significantly smaller ATFL thickness(mm,2.3±0.1 vs.2.5±0.1),CFL thickness(mm,2.2±0.2 vs.2.4±0.2)and ankle circumference(mm,16.1±2.6 vs.19.6±2.2),and larger ROM for ankle plantar flexion(38.1°±4.1° vs.31.3°±3.2° and dorsiflexion activities(32.1°±4.2° vs.24.7°±5.5°)(both P<0.05).The SF-36 also showed much better scores in the experimental group for the 8 items of bodily pain,social functioning,mental health,physical functioning,role physical,role emotional,general health,and vitality(all P<0.05).Conclu-sion Combined external application of traditional Chinese medicine hot pack is more effective in the treatment of AAS caused by naval military training than diclofenac diethylamine alone,which can improve ankle joint function,quality of life,and clinical efficacy.
