Predictive factors and prevention strategies for postoperative hydrocephalus following resection of tumors in the trigone of the lateral ventricle
Objective To investigate the predictive factors and preventive strategies for postoperative hydrocephalus following resection of tumors in the trigone of the lateral ventricle. Methods A total of 186 patients(96 males)with underwent lateral ventricle trigonal tumor resection were selected from The First Affiliated Hospital of Air Force Medical University from July 2013 to June 2023.There were 90 females,aged(39.4±10.9)years old, ranging in age from 24 to 64 years old.Hydrocephalus was divided into hydrocephalus group(n=48)and hydrocephalus free group(n=138)according to whether hydrocephalus was complicated within 14 days after surgery.Clinical data of the two groups were collected, and the influencing factors of hydrocephalus complicated by lateral ventricle triangle tumor were analyzed by Logistic regression model. Results Multivariate Logistic regression analysis showed that the degree of tumor resection, radiotherapy, preoperative hydrocephalus, surgical approach, perifocal edema, intraoperative massive ventricular hemorrhage, postoperative meningitis and postoperative fever were risk factors for postoperative hydrocephalus, and postoperative cerebrospinal fluid shunt was protective factor for postoperative hydrocephalus in lateral ventricle triangle. Conclusion The degree of tumor resection, radiotherapy, preoperative hydrocephalus, surgical approach, perifocal edema, intraoperative large amount of IVH,postoperative meningitis, and postoperative fever are important predictors of postoperative hydrocephalus after lateral ventricle triangle tumor.Protective strategies should be targeted and cerebrospinal fluid shunt should be performed in time.
