摘要
复杂性肛瘘患者肛周反复发生破溃及感染,严重影响生活质量,由于治疗难度较大,需要手术医师权衡如何在提高手术成功率的同时降低术后大便失禁风险.近年来,经肛括约肌间切开术(transanal opening of intersphincteric space,TROPIS)作为一种保留括约肌的手术方式逐渐应用于复杂性肛瘘的治疗.本文将从TROPIS治疗复杂性肛瘘的理论基础及操作流程、有效性及安全性、应用范围等方面进行综述,以期为复杂性肛瘘的治疗提供参考.
Abstract
Patients with complex anal fistula experience recurrent perianal ulceration and infection,which seriously af-fects their quality of life.Due to the difficulty of treatment,surgeons need to weigh how to improve the success rate of surgery while reducing the risk of postoperative fecal incontinence.In recent years,transanal opening of intersphincteric space(TROPIS)has gradually been applied to the treatment of complex anal fistula as a sphincter-preserving surgical ap-proach.This article will review the theoretical basis and operational procedures,effectiveness and safety,and application scope of TROPIS in the treatment of complex anal fistula,aiming to provide a reference for the treatment of complex anal fistula.
