Effects of high thoracic epidural anesthesia on cardiac function and expression of β3-adenoreceptor in rats with heart failure induced by myocardial infarction
AIM: To investigate the effect of high thoracic epidural anesthesia on cardiac function and expression of β3-adrenoceptor in rats with myocardial infarction induced heart failure. METHODS: Forty-eight rats were randomly divided into LAD group ( n = 36 ),in which the anterior descending branches of left coronary artery were ligated,and S group (n = 12) of sham-operation. LAD group was randomly subdivided into high thoracic epidural anesthesia (HTEA) group and congestive heart failure (CHF) group (n = 14,each group). A PE-10 tuber was implanted in the rats through T2 -T3 epidural cavity in the caudal direction 4 weeks later. 9 g/L normal sodium 100 μL/kg was injected to epidural cavity twice a day separately in S group and CHF group,and 1. 25 g/L bupivacaine 100 μL/kg was injected to epidural cavity twice a day in HTEA group. Epidural injection started 24 h after the epidural surgery and continued for 4 weeks. Thechange of cardiac function was then observed by measuring left ventricular end diastolic dimension (LVEDd),left ventricular end systolic dimension ( LVESd),ejection fraction ( EF) and fractionalshortening (FS). The ratio of heart weight to body weight (HW/BW) and the ratio of left ventricular weight to body weight ( LVW/BW) were measured. β3-adrenoceptor mRNA levels were detected with reverse transcription-polymerase chain reaction (RT-PCR). RESULTS: LVEDd and LVESd significantly decreased while EF and FS significantly increased in HTEA group compared with those in CHF group (both,P < 0.01). The ratios of HW/BW and LVW/BW increased significantly in CHF group compared with those in S group (P < 0. 01). β3-AR and eNOS mRNA levels significantly decreased in THEA group compared with those in CHF group. CONCLUSION: HTEA ameliorates cardiac function and depresses the left ventricular remodeling in rats with heart failure induced by myocardial infarction,which may result from the decreases of β3AR and eNOS mRNA expression in rats of heart failure.
