Clinical research on the therapeutic effect of garlic moxibustion and herb fumigate treat pachynsis pleurae with tuberculous pleurisy
AIM:To compare the expression of adenosine deaminase (ADA) and tumor necrosis factor-alpha (TNF-α) in pleural effussion in cases with tuberculous pachynsis pleurae and malignant tumor,and the therapeutic effect of garlic moxibustion and herb fumigate treat tuberculous pachynsis pleurae. METHODS: Sixty-two cases of tuberculous ( TB) pleurisy with pleural effusion and pachynsis pleurae,31 cases of malignant tumor with pleural effusion were selected. The activity of ADA was detected by colorimetric method and the expression of TNF-α was measured by ELISA in the 2 groups pleural effusions. The treatment group 32 cases were given garlic moxibustion and herb fumigate,five times a week,thirty minutes a time. Four weeks was a course of treatment. Control group 30 cases. The 2 groups were received the same anti-tuberculosis drugs regiment. All cases were checked by chest spiral CT to analyze the improvement of pachynsis pleurae after 4 weeks treatment. RESULTS: The activity of ADA and the expression of TNF-α in tuberculous effusion were significantly higher than that of malignant effusion (t = 27. 62 -19.25,P<0. 01). There was a significantly positive correlation between the ADA activity and the TNF-α expression in tuberculous pleural effusion and pachynsis pleurae ( r = 0.536 - 0. 613,P <0.01). The pachynsis pleurae was improved pre-treatment and post-treatment in treatment group (t = 4. 92,P < 0. 01). The control group was not improved (t = 1. 51,P > 0. 1). The improvement of pachynsis pleurae was better in treatment group than that of control group after 4 weeks (t =4. 59,P<0. 01). CONCLUSION: There was a positive correlation between the ADA activity and TNF-α expression and pachynsis pleurae in tuberculous pleurisy. Garlic moxibustion and herb fumigate can relieve the degree of pachynsis pleurae. The expression of ADA and TNF-α are higher in tuberculous group compare with malignant tumor group.
