Serum concentration changes of cytokines TNF-α,IL-6 in patients undergoing cardiac valve replacement and the effects of Tanreqing injection
AIM:To study the changes of serum concentration of cytokines IL-6,TNF-α during cardiopulmonary bypass in patients undergoing heart valve replacement and the effects of Tanreqing injection on them.METHODS:Thirty patients undergoing valve replacement were randomly assigned to Tanreqing group and contml group(15 in each group).In Tanreqing group,intravenous drip of 20 mL of Tanreqing injection were given at night prior to operation and before CPB,respectively,while in control group 0.9% NaCl solution was given as a placebo.Serial blood samples were collected at the following intervals respectively:Prior to CPB,40 min after CPB and 0,2,6,24 hafterthe end of CPB.We measured the serum levels of TNF-α,IL-6 by Enzyme Linked Immunosorbent Assay(ELISA)method.RESULTS:The serum levels of TNF-α,IL-6 after CPB were significantly higher than that before CPB(P<0.01).The serum levels of TNF-α reached the peak at 2 h after the end of CPB,and that still maintained a high level at 24 h after the end of CPB.The serum levels of IL-6 reached the peak at 6 h after the end of CPB,then began to decrease but still higher than the concentration before CPB significantly.The concentrations of TNF-α and IL-6 were correlated positively to the time of CPB and ACC(P<0.01).The serum levels of TNF-α and IL-6 in Tanreqing group were significantly lower than that in control group(P<0.01).CONCLUSION:The study demonstrated that there were increases of serum proinflammatory cytokines TNF-α,IL-6 during CPB in patients undergoing valve replacement.The time of CPB and ACC influenced release of TNF-α and IL-6.Tanreqing injection could reduce the release of TNF-α,IL-6,so Tanreqing injection could relieve degree of SIRS during CPB.
