Effects of hypertonic sodium chloride hydroxythel starch 40 injection on cerebral glucose extraction rate and V-A lactic acid difference of cerebral tumor patients
AIM:To observe the effect of hypertonic sodium chloride hydroxythel starch 40 injection(HH40)on Cerebral glucose extraction rate and V-A lactic acid difference of cerebral tumor patients. METHODS:Sixty ASAI-II patients with cerebral tumor undergoing elective craniotomy were randomly divided into three groups:Group HH40 A(HH40 4 mL/kg), Group HH40 B (HH40 2 mL/kg)and Group M(mannitol 4 mL/kg). All patients were accepted general anesthesia. After the exhalant concentration of sevoflurane reached 1.2 MAC, experimental fluid were dripped within 15 min. The MAP, HR and CVP were monitored before the infusion(T_0)and 15, 30, 60, 120 min after the infusion(T_1-T_4). Blood samples were collected from hulbus venae jugularis and radial artery at five time points to make blood gas analysis. RESULTS:There is no significant difference in MAP,HR and CVP. The Cerebral glucose extraction rate of HH40A group is lower than HH40B and mannitol group. There is significant difference between HH40A group and mannitol group at T_1-T_4 time points, between HH40A group and HH40B group at T_2-T_4 time points. V-A lactic acid difference of HH40A group is lower than mannitol group. There is significant difference between HH40A group and mannitol group at T_1-T_4 time points. HH40A group and HH40B group have no signifieate difference. CONCLUSION:HH40 can degrade cerebral glucose extraction rate and reduce V-A lactic acid difference . It outstrips mannitol on decreasing oxygen metabolism.
