高压氧治疗对外伤性硬膜下积液发生、发展的回顾性分析

目的:探讨高压氧(HBO)对外伤性硬膜下积液(TSE)发生、发展的影响,以及HBO对TSE的治疗作用,并分析其作用机制.方法:统计颅脑损伤后TSE总的发生率;比较颅脑损伤后早期HBO治疗(A组)和未行HBO治疗(B组)的TSE的发生情况;比较早期发现TSE后行了HBO治疗(C组)和未行HBO治疗(D组)的临床转归情况;比较钻孔引流+HBO(E组)和单纯钻孔引流(F组)治疗有手术指征的TSE的临床疗效.结果:颅脑损伤后TSE总的发生率为7.35%;早期HBO治疗组(A组)TSE的发生率为3.99%,同期未行HBO治疗组(B组)发生率为6.90%,两者比较有统计学差异(P<0.01);早期发现TSE行了HBO治疗组(C组)总有效率和治愈率为90.9l%和52.12%,未行HBO治疗(D组)总有效率和治愈率仅为77.86%和24.43%,两组比较差异显著(P<0.01);做了钻孔引流的TSE患者中,钻孔引流+HBO组(E组)总有效率和治愈率为93.10%和45.98%,单纯钻孔引流组(F组)则为80.77%和28.85%,两组比较筹异显著(P<0.05).结论:HBO治疗能有效阻止TSE的发生,并对TSE具有明显治疗效果,可作为治疗TSE的常规手段.
Effect of hyperbaric oxygenation on the genesis and development of traumatic subdural effusion
AIM:To discuss hyperbaric oxygen(HBO)for traumatic subdural effusion(TSE)genesis,development impact,as well as HBO on the TSE therapeutic effect and to analyze its mechanism.METHODS:To add up the total incidence rate of TSE after craniocerebral injury;To compare the incidence of TSE bewteen earlier HBO therapy treatment after injury(group A)and without HBO treatment(group B);To compare the clinical prognosis between HBO therapy treatment for earlier discoverable TSE (group C)and without HBO treatment(group D);To compare curative effect between trepanation and drainage+HBO therapy treatment(E group)and simple trepanation and drainage treatment(F group)for TSE of operative indication.RESULTS:The total incidence rate of TSE after craniocerebral injury is 7.35%.Its incidence rate of A group is 3.99%and that of B group is 6.90%,these two group have significant difference(P<0.01).The total effective and healing rate of C group is 90.91% and 52.12%,but that of D group is only 77.86% and 24.43%,these two group have significant difference(P<0.01).The total effective and healing rate of E group is 93.10% and 45.98%,but that of F group is 80.77% and 28.85%,these two group have significant difference(P<0.05).CONCLUSION:HBO therapy Call validly prevent the incidence of TSE and have obvious therapeutic efficacy for it,and it should be considered a conventional therapy method for TSE.
