中药胰必清在急性胰腺炎中的应用

摘要
目的 探讨中药胰必清在中西医结合治疗急性胰腺炎中的临床疗效.方法 回顾武汉大学中南医院2010～2016年间收治的90例急性胰腺炎病人,分为使用中药的研究组48例,男性26例,女性22例,年龄42～78岁;未使用中药的对照组42例,男性23例,女性19例,年龄41～74岁.研究组应用中药胰必清汤剂口服,并结合胃肠减压、生长抑素、抗炎、补液、维持水电解质及酸碱平衡等综合治疗,对照组采用胃肠减压、生长抑素、抗炎、补液、维持水电解质及酸碱平衡等常规治疗.比较分析两组病人的临床疗效.结果 应用中药胰必清的研究组与对照组比较治疗效果更显著(P＜0.05).研究组病人入院后腹痛缓解时间、恶心呕吐缓解时间、发热开始缓解时间均较短;研究组住院费用明显少于对照组;治疗后第3天行视觉模拟评分法(visual analogue scale/score,VAS)进行疼痛评分,研究组的疼痛程度明显小于对照组(P＜0.05);研究组病人出现胰腺假性囊肿发生率明显低于对照组(P＜0.05);研究组血清C反应蛋白(CRP)、血清降钙素原(PCT)、血淀粉酶、尿淀粉酶、血白细胞恢复时间均短于对照组(P＜0.05).结论 对于急性胰腺炎病人,在采用常规西医治疗的基础上,加用中药胰必清治疗,可明显缓解病人的腹痛、呕吐、发热症状,提高机体免疫力,减少并发症发生率,提高了病人的生活质量,值得临床推广应用.
Abstract
Objective To discuss the clinical curative effect of Yibiqing in the treatment of acute pancreatitis.Methods We selected 90 patientswith acute pancreatitis in our hospitalfrom 2010 to 2016.The study group was givenYibiqing,combined with gastrointestinal decompression,somatostatin,anti-inflammation,rehydration.The control group was given the conventional treatment only.The clinical efficacy was compared betweenthe two groups.Results Yibiqinghad an obvious therapeutic effect as compared with control group (P＜0.05).In the study group,the time of abdominal pain,nausea,vomiting and fever was significantly shorten than in the control group after admission.The hospitalization expenses weresignificantly recued in the study group as compared with those in the control group.The pain scores and the incidence of pancreatic pseudocystsin the study group weresignificantly lower than those in the control group (P＜0.05).The serum CRP,PCT,amylase,blood leukocytes and urine amylase returned to the normal levels more quickly in the study group than in the control group (P＜0.05).Conclusions For patients with acute pancreatitis,combination of Yibiqing with routine western medication couldsignificantly relieve the abdominal pain,vomiting and fever,enhance immunity,reduce the incidence of complications,and improve quality of life.It was worth popularizing and applying in the clinical practice.
