乌司他丁联合亚低温对心肺复苏患者血清TLR4/NF-κB指标的影响

目的 分析乌司他丁联合亚低温对心肺复苏(CPR)患者Toll样受体 4/核因子-κB(TLR4/NF-κB)指标的影响.方法 采用前瞻性研究法,对2020年6月至2022年2月期间西安国际医学中心医院收治的120例CPR救治成功后昏迷患者展开研究,采用简单随机法将其分为四组,各组均30例.A组给予常规对症支持,B组给予常规+乌司他丁治疗,C组给予常规+亚低温治疗,D组给予常规+乌司他丁+亚低温治疗.比较四组治疗前后炎症反应情况、心功能情况、脑功能情况、TLR4和NF-κB水平.结果 与A组相比,B组、C组和D组白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肌钙蛋白(cTnI)和左心室舒张末期内径(LVEDD)水平均降低,差异有统计学意义(P<0.05),心脏左室射血分数(LVEF)水平、格拉斯哥昏迷评分(GCS)、神经功能综合评分(NFCS)、TLR4和NF-κB水平均升高,差异有统计学意义(P<0.05).与D组相比,B组、C组IL-6、IL-8、cTNI、LVEDD、TLR4、NF-κB水平均升高,差异有统计学意义(P<0.05),LVEF、GCS评分、NFCS评分均降低,差异有统计学意义(P<0.05).结论 乌司他丁联合亚低温治疗能够改善炎症反应,发挥心功能和脑功能保护作用,可能与其负反馈调节TLR4/NF-κB有关.
Effect of ulinastatin combined with subhypothermia on serum TLR4/NF-κB indicators in patients undergoing cardiopulmonary resuscitation
Objective To analyze the effects of ulinastatin combined with mild hypothermia on toll-like receptor 4/nuclear factor-KB(TLR4/NF-κB)indexes in patients with cardiopulmonary resuscitation(CPR).Methods A prospective study was conducted on 120 patients with coma after successful CPR treat-ment in Xi'an International Medical Center Hospital from June 2020 to June 2022.The patients were divided in-to four groups by simple random method,with 30 cases in each group.Group A received routine symptomatic support,Group B received routine + ulinastatin treatment,Group C received routine + mild hypothermia treat-ment,and Group D received routine + ulinastatin + mild hypothermia treatment.The inflammatory response,cardiac function,brain function,TLR4 and NF-κB levels before and after treatment among the four groups were compared.Results Compared with Group A,the levels of interleukin 6(IL-6),interleukin 8(IL-8),troponin(cTnI)and left ventricular end diastolic diameter(LVEDD)in Group B,Group C and Group D were significantly lower(P<0.05).The levels of left ventricular Ejection fraction(LVEF),Glasgow coma scale(GCS),comprehensive neurological score(NFCS),TLR4 and NF kB were significantly increased(P<0.05).Compared with Group D,the levels of IL-6,IL-8,cTNI,LVEDD,TLR4,and NF-κB in Group B and Group C increased with statistical significance(P<0.05),while LVEF,GCS scores,and NFCS scores de-creased with statistical significance(P<0.05).Conclusion Ulinastatin combined with mild hypothermia treat-ment can improve inflammatory response and exert protective effects on cardiac and brain function,which may be related to its negative feedback regulation of TLR4/NF-κB.
