摘要
目的 分析研究碳酸司维拉姆联合高通量透析对维持性血液透析(MHD)患者微炎症、肾功能及心肌肌钙蛋白T(cTnT)影响.方法 选取2021年1月至2023年1月黄山市人民医院收治的98例进行MHD患者作为研究对象,根据不同的治疗方案将患者分成研究组(n=49)和对照组(n=49).其中对照组给予常规普通透析治疗,研究组给予碳酸司维拉姆联合高通量透析治疗,连续治疗3个月.比较两组治疗前、治疗后1个月及治疗后3个月的血清钙、磷、炎症因子(CRP、IL-6、PCT)、肾功能(Scr、BUN)及cTnT水平;比较两组不良反应发生率.结果 两组治疗1个月、3个月的血清钙、磷、CRP、IL-6、PCT、Scr、BUN、cTnT水平均优于治疗前,差异均具有统计学意义(P<0.05);研究组治疗1个月、3个月的血清钙、磷、CRP、IL-6、PCT、Scr、BUN、cTnT水平均优于对照组,差异均具有统计学意义(P<0.05);两组不良反应发生率相比较,差异无统计学意义(χ2=0.089,P>0.05).结论 碳酸司维拉姆联合高通量透析能够降低MHD患者的微炎症状态、改善肾功能与减少心肌损伤.
Abstract
Objective To analyze the effects of sevelamer carbonate combined with high-through-put dialysis on microinflammation,renal function and cardiac troponin T(cTnT)in maintenance hemodialysis(MHD)patients.Methods 98 cases of MHD patients admitted to Huangshan People's Hospital from January 2021 to January 2023 were selected as the study objects,and the patients were divided into the study group(n=49)and the control group(n=49)according to different treatment regimens.The control group was given conventional dialysis treatment,and the study group was given sevelamer carbonate combined with high-throughput dialysis treatment for 3 months.The levels of serum calcium,phosphorus,inflammatory factors(CRP,IL-6,PCT),renal function(Scr,BUN)and cTnT were compared between the two groups before treatment,1 month after treatment and 3 months after treatment.The incidence of adverse reactions was com-pared between the two groups.Results The levels of serum calcium,phosphorus,CRP,IL-6,PCT,Scr,BUN and cTnT in 2 groups after 1 month and 3 months of treatment were better than before treatment,and the differences were statistically significant(P<0.05).The levels of serum calcium,phosphorus,CRP,IL-6,PCT,Scr,BUN and cTnT in the study group after 1 month and 3 months of treatment were better than those in the control group,and the differences were statistically significant(P<0.05).There was no significant dif-ference in the incidence of adverse reactions between the two groups(χ2=0.089,P>0.05).Conclusion Sevelamer carbonate combined with high-throughput dialysis can reduce microinflammatory status,improve re-nal function and reduce myocardial injury in MHD patients.
