目的 探究脊髓型颈椎病(CSM)患者临床表现与磁共振(MRI)及弥散张量成像(DTI)量化值的相关性.方法 选取2020年1月至2022年12月期间河北中石油中心医院65例CSM患者作为研究对象,根据日本骨科协会(JOA)评分将患者分为轻度组、中度组、重度组,均行MRI及DTI检查,分析各组患者病变节段最狭窄层面脊髓MRI表现,及DTI参数[表观弥散系数(ADC)、各向异性分数(FA)]、本征值(λ1值、λ2值、λ3值),采用Pearson相关性分析ADC值、FA值及本征值与JOA评分的相关性.结果 重度组患者MRI T2WI高信号占比高于中度组和轻度组(χ2=35.033,P<0.05),中度组与轻度组MRI信号表现分布差异无统计学意义(χ2=0.931,P>0.05);三组病变节段ADC值比较:重度组>中度组>轻度组(F= 40.042,P<0.05);FA值比较:重度组<中度组<轻度组,组间差异均有统计学意义(F=33.145,P<0.05);三组病变节段λ1值比较差异无统计学意义(F=0.344,P>0.05),三组λ2值与λ3值比较,均重度组>中度组>轻度组,组间差异有统计学意义(F=25.296,62.994,P<0.05);Pearson相关性分析显示,ADC值与JOA评分呈负相关(P<0.05),FA值与JOA评分呈正相关(P<0.05).结论 MRI常规扫描及DTI量化值可反映CSM患者脊髓病变节段损伤程度,其中ADC值和FA值与患者的临床症状严重程度呈线性相关性.
Correlation between clinical manifestations and quantitative values of MRI and DTI in pa-tients with cervical spondylotic myelopathy
Objective To explore the correlation between clinical manifestations and quantitative values of magnetic resonance imaging(MRI)and diffusion tensor imaging(DTI)in patients with cervical spondylotic myelopathy(CSM).Methods 65 patients with CSM in Hebei China Petroleum Central Hospital from January 2020 to December 2022 were selected as the research subjects.According to Japanese Orthopedic Association(JOA)score,the patients were divided into the mild group,the moderate group and the severe group.MRI and DTI were performed on all patients.MRI manifestations of spinal cord at the narrowest level of the lesion segment,as well as DTI parameters[apparent diffusion coefficient(ADC),fractional anisotropy(FA)]and eigenvalues(λ1 value,λ2 value,λ3 value)were analyzed among the groups.Pearson correlation was used to analyze the correlation between ADC,FA value and eigenvalues and JOA score.Results The proportion of high signal on MRI T2WI in the severe group was higher than that in the moderate group and the mild group(χ2=35.033,P<0.05),but there was no statistical difference in the distribution of MRI signal mani-festation between the moderate group and the mild group(χ2=0.931,P>0.05).Comparison of ADC values of diseased segments in three groups:severe group>moderate group>mild group(F=40.042,P<0.05).Com-parison of FA values:severe group<moderate group<mild group,the differences between groups were statis-tically significant(F=33.145,P<0.05).There was no statistical significance in λ1 value of lesion segment among the three groups(F=0.344,P>0.05),and the λ2 value and λ3 value were shown as severe group>mod-erate group>mild group,and the differences between groups were statistically significant(F=25.296/62.994,P<0.05).Pearson correlation analysis showed that the ADC value was negatively correlated with the JOA score(P<0.05),and the FA value was positively correlated with the JOA score(P<0.05).Conclusion Routine MRI scans and DTI quantitative values can reflect the degree of spinal cord segment damage in patients with CSM,and the ADC value and FA value are linearly related to the severity of the patient's clinical symptoms.
