摘要
目的 探究皮肤恶性黑色素瘤(CMM)血清miR-211、miR-128水平及与疗效关系.方法 选取2021年7月至2022年6月期间河南中医药大学第三附属医院收治的120例CMM患者为CMM组,选取同期90例非肿瘤皮肤病患者为对照组,比较两组血清miR-211、miR-128表达水平,分析血清miR-211、miR-128表达与CMM临床病理特征关系;3个月后,根据CMM组患者实体瘤疗效分为缓解组和未缓解组,分析血清miR-211、miR-128表达与患者疗效关系.结果 CMM组血清miR-211、miR-128表达量低于对照组,差异有统计学意义(t=18.186、48.871,P均<0.05).不同病理类型、分化程度、淋巴结转移、临床分期、Ki-67指数患者血清miR-211表达比较差异有统计学意义(F或t=13.209、3.044、5.601、7.996、3.748,P均<0.05),不同分化程度、淋巴结转移、临床分期患者血清miR-128表达比较差异有统计学意义(t=5.940、9.592、12.895,P均<0.05).缓解组与未缓解组病理类型、分化程度、有无淋巴结转移、临床分期、Ki-67指数水平、是否手术比较差异均有统计学意义(χ2=8.062、19.171、7.333、14.436、5.025、4.309,P均<0.05);治疗后,缓解组血清miR-211、miR-128表达均高于未缓解组,差异有统计学意义(t=13.926、8.849,P均<0.05).多因素Logistic回归分析显示,低分化(OR=2.275,95%CI:1.027～5.042)、淋巴结转移(OR=2.643,95%CI:1.243～5.622)、临床分期Ⅲ～Ⅳ期(OR=3.022,95%CI:1.348～6.777)、miR-211低表达(OR=2.208,95%CI:1.217～4.006)、miR-128低表达(OR=2.375,95%CI:1.086～5.192)是影响患者疗效的危险因素(P<0.05).结论 CMM患者血清miR-211、miR-128表达水平明显降低,两指标均与患者临床病理特征及疗效有关,具有一定参考价值.
Abstract
Objective To explore the levels of serum miR-211 and miR-128 and the relationship with efficacy in cutaneous malignant melanoma(CMM).Methods 120 patients with CMM admitted to the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from July 2021 to June 2022 were selected as the CMM group,and 90 patients with non-tumor skin diseases during the same period were enrolled as the control group.The expression levels of serum miR-211 and miR-128 were compared between the two groups,and the relationship between expressions of serum miR-211 and miR-128 and clinicopathologi-cal characteristics of CMM was analyzed.After 3 months,the patients in the CMM group were divided into the remission group and the non-remission group according to the efficacy of solid tumors.The relationship be-tween expressions of serum miR-211 and miR-128 and efficacy of patients was analyzed.Results The expres-sion levels of serum miR-211 and miR-128 in the CMM group were lower than those in the control group(t= 18.186,48.871,P<0.05).There was a statistically significant difference in the expression of serum miR-211 among patients with different pathological types,differentiation degrees,lymph node metastasis,clinical stag-es and Ki-67 index(F or t=13.209,3.044,5.601,7.996,3.748,P<0.05).There was a statistical significance in the expression of serum miR-128 among patients with different differentiation degrees,lymph node metasta-sis and clinical stages(t=5.940,9.592,12.895,P<0.05).There were statistical differences in pathological type,differentiation degree,presence or absence of lymph node metastasis,clinical staging,Ki-67 index and presence or absence of surgery between the remission group and the non-remission group(χ2=8.062,19.171,7.333,14.436,5.025,4.309,P<0.05).The expression levels of serum miR-211 and miR-128 in the remission group after treatment were higher compared to the non-remission group(t=13.926,8.849,P<0.05).Multivari-ate Logistic regression analysis showed that poor differentiation(OR=2.275,95%CI:1.027～5.042),lymph node metastasis(OR=2.643,95%CI:1.243～5.622),clinical stageⅢ～Ⅳ(OR=3.022,95%CI:1.348～6.777),low expression of miR-211(OR=2.208,95%CI:1.217～4.006),and low expression of miR-128(OR=2.375,95%CI:1.086～5.192)were risk factors affecting the efficacy of patients(P<0.05).Conclusion The expres-sion levels of serum miR-211 and miR-128 in patients with CMM are significantly decreased and are related to clinicopathological characteristics and efficacy of patients and have certain reference value.
