摘要

目的 探讨慢性心力衰竭(CHF)患者肾素(REN)、血管紧张素Ⅱ(AngⅡ)及脑钠肽(BNP)变化及其与心功能的关系.方法 选取2019年2月至2023年2月金寨县人民医院收治的84例CHF患者的临床资料(CHF组),另取同期在本院进行体检的66名健康人作为健康对照组.分析两组REN、AngⅡ及BNP水平;以及不同心功能分级患者REN、AngⅡ、BNP水平及心功能指标;采用Pearson相关性分析法分析REN、AngⅡ及BNP与心功能指标的相关性;采用ROC曲线评估NE、AngⅡ及BNP水平对CHF的预测价值.结果 CHF组REN、AngⅡ及BNP水平显著高于对照组,差异具有统计学意义(P<0.05).不同心功能分级CHF患者REN、AngⅡ、BNP水平比较:NYHAⅡ级组<NYHAⅢ级组<NYHAⅣ级组,差异具有统计学意义(P<0.05).不同心功能分级CHF患者左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)比较:NYHAⅡ级组<NYHAⅢ级组<NYHAⅣ级组,左室射血分数(LVEF)比较:NYHAⅡ级组>NYHAⅢ级组>NYHAⅣ级组,差异具有统计学意义(P<0.05).经相关分析表明,REN、AngⅡ、BNP与LVEDD、LVESD呈正相关,与LVEF呈负相关关系,且REN、AngⅡ、BNP与心功能分级均呈正相关(P<0.05).ROC结果显示,REN、AngⅡ及BNP三者联合(REN+AngⅡ+BNP)诊断为0.901,明显高于单独诊断(P<0.05).结论 CHF患者REN、AngⅡ及BNP水平均异常增高,且三个指标与患者心功能分级呈显著相关性,上述因子有望成为临床诊断CHF及评估患者心功能分级的可靠指标.

Abstract

Objective To investigate the changes of renin(REN),angiotensin(AngⅡ)and brain natriuretic peptide(BNP)in patients with chronic heart failure(CHF)and its relationship with cardiac function. Methods The clinical data of 84 patients with CHF admitted to Jinzhai County People's Hospital from February 2019 to February 2023 were selected(CHF group),another 66 healthy people who underwent physical examination in our hospital during the same period were selected as the healthy control group. We analyzed the levels of REN,Ang Ⅱ and BNP in the two groups;and the levels of REN,Ang Ⅱ,BNP and cardiac function of patients with different cardiac function grades were analyzed by variance;the correlations of REN,AngⅡand BNP with cardiac function indexes were analyzed by pearson correlation analysis;and ROC curve was used to evaluate the predictive value of REN,AngⅡand BNP levels for CHF. Results The levels of REN, Ang Ⅱ and BNP in CHF group were significantly higher than those in control group ,and the difference was statistically significant(P<0.05). Comparison of the levels of REN,Ang Ⅱand BNP in CHF patients with different cardiac function grades:NYHA Ⅱgroup<NYHA Ⅲ group<NYHA Ⅳ group,and the difference was statistically significant(P<0.05). Comparison of left ventricular end-diastolic diameter(LVEDD)and left ventricular end-systolic diameter (LVESD) in CHF patients with different cardiac function grades:NYHAⅡ group<NYHA Ⅲ group<NYHA Ⅳ group;Comparison of left ventricular ejection fraction(LVEF):NYHA Ⅱ group>NYHAⅢgroup>NYHA Ⅳ group,and the difference was statistically significant(P<0.05). The correlation analysis showed that REN,AngⅡ and BNP were positively correlated with LVEDD and LVESD,but were negatively correlated with LVEF,and REN,Ang Ⅱ and BNP were positively correlated with cardiac function grading(P<0.05). ROC results showed that the AUC of REN Ang Ⅱ and BNP. The AUC of REN+Ang Ⅱ +BNP was 0.901,which was significantly higher than that of single diagnosis(P<0.05). Conclusion The levels of REN,Ang Ⅱand BNP in CHF patients were abnormally increased,and the three indexes were significantly correlated with cardiac function grading ,these factors are expected to become reliable indicators for clinical diagnosis of CHF and evaluation of cardiac function classification.
