血清FGF21、Sfrp5、miRNA-21对冠心病经皮冠状动脉介入治疗术后支架内再狭窄的预测价值

目的 探讨血清成纤维细胞生长因子21(FGF21)、分泌型卷曲蛋白5(Sfrp5)、微小RNA-21(miRNA-21)对冠心病(CHD)经皮冠状动脉介入治疗术(PCI)后支架内再狭窄(ISR)的预测价值.方法 选择商水县人民医院2021年1月至2022年9月间行PCI手术治疗的CHD患者218例为CHD组,同期非心血管疾病的住院患者97例为对照组,比较两组FGF21、Sfrp5及miRNA-21;CHD组术后随访6个月,以是否发生ISR分为ISR组(n=56)及无ISR组(n=162);分析CHD患者行PCI手术后发生ISR的影响因素,分析FGF21、Sfrp5及miRNA-21对ISR的预测价值.结果 CHD组FGF21、miRNA-21均高于对照组,Sfrp5水平低于对照组,差异有统计学意义(t=11.196、15.030、13.685,P<0.05);ISR组低密度脂蛋白胆固醇(LDL-C)、FGF21、miRNA-21均高于无ISR组,Sfrp5水平低于无ISR组,差异有统计学意义(t=7.093、7.216、6.388,P<0.05),ISR组吸烟、合并糖尿病比率高于无ISR组,差异有统计学意义(x2=6.015、11.295,P<0.05);多因素Logistic回归分析显示,LDL-C、合并糖尿病、FGF21、Sfrp5、miRNA-21是CHD患者行PCI术后发生ISR的影响因素(P<0.05);ROC曲线分析显示,FGF21、Sfrp5、miRNA-21三者单独及联合检测预测PCI术后发生ISR的AUC分别为0.772、0.758、0.763、0.905(P<0.05).结论 血清FGF21、Sfrp5、miRNA-21在行PCI手术后发生ISR的CHD患者中表达异常,联合检测三者水平对于预测CHD患者PCI术后发生ISR具有较好的应用价值.
The predictive value of serum FGF21,Sfrp5 and miRNA-21 for in-stent restenosis after percutaneous coronary intervention in patients with coronary heart disease
Objective To investigate the predictive value of serum fibroblast growth factor 21 (FGF21),secreted frizzled protein 5(Sfrp5)and microRNA-21(miRNA-21)for in-stent restenosis(ISR)after percutaneous coronary intervention(PCI)in patients with coronary heart disease(CHD). Methods A total of 218 CHD patients who underwent PCI at Shangshui County People's Hospital from January 2021 to September 2022 were selected as the CHD group,and 97 hospitalized patients without cardiovascular disease during the same period were selected as the control group. The levels of FGF21,Sfrp5 and miRNA-21 were compared between the two groups. The CHD group was followed up for 6 months after operation,and was divided into the ISR group(n=56)and the non-ISR group(n=162)based on whether ISR occurred or not. The aim was to analyze the influencing factors of ISR after PCI in CHD patients,and to determine the predictive value of FGF21,Sfrp5 and miRNA-21 for ISR. Results FGF21 and miRNA-21 levels were higher in the CHD group compared to the control group,while the level of Sfrp5 was lower than that in the control group(t=11.196, 15.030,13.685,P<0.05). Low density lipoprotein cholesterol(LDL-C),FGF21 and miRNA-21 in the ISR group were higher than those in the non-ISR group,and the level of Sfrp5 was lower than that in the non-ISR group,the differences were statistically significant(t=7.093,7.216,6.388,P<0.05). The rates of smoking and diabetes mellitus in the ISR group were higher than those in the non-ISR group,the differences were statistically significant(x2=6.015,11.295,P<0.05). Multivariate logistic regression analysis showed that LDL-C,diabetes mellitus,FGF21,Sfrp5 and miRNA-21 were the influencing factors for ISR after PCI in CHD patients(P<0.05). The ROC curve analysis showed that the AUC for FGF21,Sfrp5 and miRNA-21 alone and combined detection in predicting ISR after PCI were 0.772,0.758,0.763 and 0.905,respectively. Conclusion Serum FGF21,Sfrp5 and miRNA-21 are abnormally expressed in CHD patients with ISR after PCI. The combined detection of the three levels has a high application value in predicting ISR after PCI in CHD patients.
