miR-92b-5p、HMGB1与2型糖尿病肾病患者肾功能的相关性

目的 探讨微小核糖核酸-92b-5p(miR-92b-5p)、血清高迁移率族蛋白B1(HMGB1)与2型糖尿病肾病患者肾功能的相关性.方法 选取2021年1月至2023年12月期间河南省驻马店市中心医院内分泌科收治的212例2型糖尿病肾病患者作为肾病组,另选取同期来本院就诊的单纯2型糖尿病患者 95 例作为 2 型糖尿病组.比较二组患者的肾功能相关指标以及miR-92b-5p、HMGB1 水平;以Pearson相关性分析评估2型糖尿病肾病患者血清miR-92b-5、HMGB1水平与肾功能的相关性;以单、多因素Logistic回归分析2型糖尿病患者并发肾病的危险因素,并绘制受试者工作特性曲线(ROC)进一步评估miR-92b-5p、HMGB1二者联合检测对2型糖尿病患者并发肾病的预测价值.结果 两组糖尿病病程、BUN、Scr、eGFR、miR-92b-5p以及HMGB1水平的比较差异有统计学意义(P<0.05);Pearson分析显示,HMGB1 与BUN、Scr呈正相关,与eGFR呈负相关(r=0.570,0.646,-0.485,P<0.05);miR-92b-5 与BUN、Scr呈负相关,与eGFR呈正相关(r=-0.526,-0.562,0.442,P<0.05).多因素回归分析显示,糖尿病病程增加、BUN、Scr、HMGB1水平升高、eGFR、miR-92b-5p水平降低均是2型糖尿病患者并发肾病的独立危险因素(P<0.05);ROC曲线分析显示,miR-92b-5p、HMGB1以及二者联合检测曲线下面积为0.898、0.932、以及0.961(P<0.05).结论 2型糖尿病肾病患者肾功能异常可能与血清中miR-92b-5p表达量下降、HMGB1表达量上升有关,二指标可作为2型糖尿病肾病的有效预测指标.
Correlation analysis of miR-92b-5p and HMGB1 with renal function in patients with type 2 diabetic nephropathy
Objective To investigate the correlation between microrNA-92b-5p(miR-92b-5p),serum high mobility group protein B1(HMGB1)and renal function in patients with type 2 diabetic nephropa-thy.Methods Choose between January 2021 and December 2023,henan zhumadian city centre hospital en-docrinology admitted during the period of 212 cases of patients with type 2 diabetic nephropathy as kidney dis-ease group,the selection to the same period of simple type 2 diabetes patients in our hospital 95 cases as type 2 diabetes mellitus group.Renal function related indexes,miR-92b-5p and HMGB1 levels were compared be-tween the two groups.The correlation between serum miR-92b-5,HMGB1 and renal function in patients with type 2 diabetic nephropathy was evaluated by Pearson correlation analysis.Multivariate Logistic regression was used to analyze the risk factors of kidney disease in patients with type 2 diabetes,and receiver operating char-acteristic curve(ROC)was plotted to further evaluate the predictive value of miR-92b-5p and HMGB1 com-bined detection in patients with type 2 diabetes with kidney disease.Results Univariate analysis showed that there were significant differences in the expression levels of BUN,Scr,eGFR,miR-92b-5p and HMGB1 be-tween the two groups.Pearson analysis showed that HMGB1 was positively correlated with BUN and Scr,but negatively correlated with eGFR(r=0.570,0.646,-0.485,P<0.05).miR-92b-5 was negatively correlated with BUN and Scr,but positively correlated with eGFR(r=-0.526,-0.562,0.442,P<0.05).Multivariate Logistic regression analysis further confirmed that insulin injection,increased BUN,Scr and HMGB1 levels,and decreased eGFR and miR-92b-5p levels were independent risk factors for nephropathy in type 2 diabetes patients(P<0.05).ROC curve analysis showed that the areas under the detection curves of miR-92b-5p,HMGB1 and their combined detection were 0.898,0.932 and 0.961(P<0.05).Conclusion The abnormal renal function in patients with type 2 diabetic nephropathy may be related to the decreased expression of miR-92b-5p and the increased expression of HMGB1 in serum.These two indicators can be used as effective predic-tors of type 2 diabetic nephropathy.
