外周血NF-κB、BDNF及NGF与硼替佐米相关周围神经病变的关系分析

目的 探讨外周血核因子κB(NF-κB)、脑源性神经营养因子(BDNF)及神经生长因子(NGF)与硼替佐米相关周围神经病变(BiPN)的关系.方法 选取2020年8月至2023年8月承德医学院附属医院收治的106例多发性骨髓瘤(MM)患者作为研究对象,所有患者均给予硼替佐米治疗.对比患者治疗前后外周血NF-κB mRNA相对表达量、血清BDNF、NGF、P75神经营养素受体(P75NTR)水平变化.治疗结束后,根据PN发生情况将患者分为PN组(n=49)和无PN组(n=57),单、多因素Logistic回归分析MM患者BiPN的影响因素,受试者工作特征(ROC)曲线分析治疗前外周血NF-κB、BDNF、NGF、P75NTR水平对MM患者硼替佐米治疗发生周围神经病变的预测价值.结果 治疗后,患者NF-κB mRNA相对表达量、P75NTR水平低于治疗前,差异有统计学意义(t=7.282、8.069,P<0.05),患者血清BDNF、NGF水平高于治疗前,差异有统计学意义(t=13.234、7.152,P<0.05).两组给药频次、治疗前NF-κB mRNA相对表达量、血清P75NTR、BDNF、NGF水平对比,差异有统计学意义(t=6.552、8.134、8.447、4.852、6.607,P<0.05).每周2次给药、治疗前NF-κB mRNA高表达、P75NTR水平升高、血清BDNF水平、NGF水平降低是MM患者硼替佐米治疗发生周围神经病变的独立危险因素(P<0.05).ROC曲线分析结果显示,NF-κB mRNA、血清BDNF、NGF、P75NTR水平联合预测MM患者BIPN的曲下线面积高于各指标单独检测(P<0.01).结论 NF-κB mRNA、血清BDNF、NGF、P75NTR水平联合检测对BIPN有一定预测价值.
Relationship between NF-κB,BDNF and NGF in peripheral blood and bortezomib related peripheral neuropathy
Objective To investigate the relationship between nuclear factor κB(NF-κB),brain-derived neurotrophic factor(BDNF)and nerve growth factor(NGF)in peripheral blood and bortezomib asso-ciated peripheral neuropathy(BiPN).Methods 106 patients with multiple myeloma(MM)admitted to the Affiliated Hospital of Chengde Medical College from August 2020 to August 2023 were selected as the study objects,all patients were treated with bortezomib.The relative expression of NF-κB mRNA in peripheral blood and the levels of BDNF,NGF and P75 neuronutrient receptor(P75NTR)in serum were compared before and after treatment.After treatment,patients were divided into PN group(n=49)and no PN group(n=57)accord-ing to the occurrence of PN.Single and multi-factor Logistic regression analysis was conducted to analyze the influencing factors of BiPN in MM patients.The predictive value of NF-κB,BDNF,NGF and P75NTR levels in peripheral blood before treatment in patients with MM treated with bortezomib peripheral neuropathy was an-alyzed by receiver operating characteristic(ROC)curve.Results After treatment,the relative expression of NF-κB mRNA and P75NTR levels were lower than before treatment(t=7.282 and 8.069,P<0.05),and the se-rum BDNF and NGF levels were higher than before treatment(t=13.234 and 7.152,P<0.05).The frequency of administration,the relative expression of NF-κB mRNA before treatment,and the levels of serum P75NTR,BDNF and NGF were compared between the two groups,and the differences were statistically sig-nificant(t=6.552,8.134,8.447,4.852 and 6.607,P<0.05).Dosing twice a week,high NF-κB mRNA expres-sion,increased P75NTR level before treatment,and decreased serum BDNF level and NGF level were inde-pendent risk factors for peripheral neuropathy in MM patients treated with bortezomib(P<0.05).ROC curve analysis showed that the combined prediction of NF-κB mRNA,serum BDNF,NGF and P75NTR levels was higher than that of each index alone(P<0.01).Conclusion The combined detection of NF-κB mRNA,serum BDNF,NGF and P75NTR levels has a certain predictive value for BIPN.
