血清D-D、MHR、UA及RDW与冠心病患者左心室重构和心功能的相关性

目的 探究血清D-二聚体(D-D)、尿酸(UA)、单核细胞/高密度脂蛋白胆固醇比值(MHR)及红细胞分布宽度(RDW)与冠心病患者左心室重构和心功能的相关性.方法 选取2022年1月至2022年11月芜湖市第二人民医院收治的行冠脉造影术适应症患者,根据冠脉狭窄程度分为冠心病组 68 例,对照组 44 例.比较两组一般资料、生化指标水平和左心室重构指标水平;采用Pearson和Spearman相关性分析四个指标与左心室重构指标和心功能分级的关系.结果 冠心病组D-D、MHR、UA、RDW、左心房内径(LAD)、左室舒张末期内径(LVEDD)、舒张末期室间隔厚度(IVSd)、舒张末左室后壁厚度(LVPWd)、左心室质量指数(LVMI)均显著高于对照组,左心室射血分数(LVEF)显著低于对照组,差异均有统计学意义(P<0.05);Pearson分析结果显示,D-D和RDW与LVEF呈负相关;D-D与LAD、LVEDD、IVSd、LVPWd、LVMI,MHR与LAD、LVEDD、LVMI,UA与LAD、LVPWd、LVMI,RDW与LAD、IVSd均呈正相关(P<0.05);Spearman分析结果显示,四个指标与心功能分级均呈正相关(P<0.05).结论 冠心病患者血清D-D、MHR、UA、RDW与左心室重构和心功能分级有一定相关性,上述指标可以用于辅助诊断与评估患者病情.
Correlation between serum D-D,MHR,UA,RDW and left ventricular remodeling,cardiac function in patients with coronary heart disease
Objective To explore the correlation between serum D-dimer(D-D),uric acid(UA),monocyte/high-density lipoprotein cholesterol ratio(MHR),red blood cell distribution width(RDW)and left ventricular remodeling,cardiac function in patients with coronary heart disease(CHD).Methods Patients undergoing coronary angiography at the Second People's Hospital of Wuhu were enrolled in the study between January and November 2022.After obtaining informed consent,their general data was collected.Based on the degree of coronary stenosis,participants were divided into the CHD group(68 cases)and the control group(44 cases).Differences in general data,the levels of biochemical indicators,and left ventricular remodeling indicators between the two groups were analyzed using t-tests and χ2-tests.The correlation between D-D,UA,MHR,RDW,left ventricular remodeling indexes,and cardiac function grading was assessed us-ing Pearson and Spearman correlation analysis.Results D-D,MHR,UA,RDW,left atrial diameter(LAD),left ventricular end-diastolic diameter(LVEDD),interventricular septal thickness at diastole(IVSd),left ventricular posterior wall depth(LVPWd),and left ventricular mass index(LVMI)in the CHD group were significantly higher than those in the control group,while left ventricular ejection fraction(LVEF)was significantly lower in the CHD group compared to the control group,and the differences are statistically significant(P<0.05).The results of Pearson correlation analysis indicated that D-D and RDW were negatively correlated with LVEF.Additionally,D-D positively correlation with LAD,LVEDD,IVSd,LVPWd,and LVMI.MHR showed a positive correlated with LAD,LVEDD,and LVMI,while UA was positively correlat-ed with LAD,LVPWd,and LVMI.RDW was found to be positively correlated with LAD and IVSd(P<0.05).The results of Spearman correlation analysis showed that D-D,MHR,UA and RDW were all positively correlated with cardiac function grading(P<0.05).Conclusion Serum D-D,MHR,UA,and RDW are cor-related with left ventricular remodeling and cardiac function grading.These markers can be used to assist in the diagnosis and evaluation of patients'conditions.
