The diagnostic value of D-dimer and C-peptide levels combined with percutaneous partial pressure of oxygen in diabetic peripheral neuropathy
Objective To explore the diagnostic value of D-dimer and C-peptide levels combined with percutaneous partial pressure of oxygen in diabetic peripheral neuropathy(DPN).Methods A total of 60 patients with type 2 diabetes combined with DPN treated at Taihe Hospital Affiliated to Wannan Medical College,Anhui Province from January 2022 to January 2023 were selected as the study group,and 60 patients with simple type 2 diabetes without DPN during the same period were included as the control group.Clinical data and biochemical indexes from both groups were collected and compared.The risk factors for peripheral neuropathy in diabetic patients were analyzed using multivariate logistic regression.The diagnostic value of D-dimer and C-peptide levels combined with transcutaneous partial pressure of oxygen(TcPO2)in diabetic pe-ripheral neuropathy was analyzed by receiver operating characteristic(ROC)curve analysis.Result The lev-els of BMI,2hC peptide after eating and TcPO2 in the study group were significantly lower than those in the control group,and the differences were statistically significant(t=2.435,6.215,5.849,P<0.05).The dura-tion of diabetes mellitus,serum uric acid(SUA),glycosylated hemoglobin(HbA1c)and D-dimer values were higher than those of control group,with statistical significance(t=6.677,5.789,5.621,5.285,P<0.05).The results of multi-factor logistics regression analysis showed that diabetes course,HbA1c,D-dimer,2hC peptide after eating and TcPO2 were independent influencing factors of peripheral neuropathy in diabetic patients(P<0.05).ROC curve analysis results showed that the area under the curve(AUC)of D-dimer,C-peptide level and percutaneous oxygen partial pressure were 0.752,0.784,0.769 and 0.861 respectively,and the diagnostic value of the combined detection of D-dimer,C-peptide level and percutaneous oxygen par-tial pressure was better than that of a single index(P<0.05).Conclusion The combined detection of serum C-peptide,percutaneous oxygen partial pressure and D-dimer in diabetic patients has a good diagnostic value for peripheral neuropathy.
