目的 探究血清醌氧化还原酶(NQO1)、C反应蛋白与载脂蛋白A1比值(CRP/ApoA1)及可溶性晚期糖基化终末产物受体(sRAGE)与原发性高血压患者冠状动脉病变的相关性.方法 分析2021年 6月至 2022年 12月期间广东药科大学附属第一医院就诊的原发性高血压患者资料 122例.根据合并冠状动脉病变与否分为单纯高血压组(77例)和冠状动脉病变组(45例),并根据Gensini评分将冠状动脉病变组患者分为轻度组(14例),中度组(18例),重度组(13例).比较两组患者血清NQO1、CRP/ApoA1、sRAGE水平差异;Pearson相关性检验分析NQO1、CRP/ApoA1、sRAGE与Gensini评分的关系;多因素Logistic回归模型分析高血压患者并发冠状动脉病变的影响因素.结果 病变组患者吸烟、合并糖尿病占比高于单纯高血压组(x2=22.215、9.872,P<0.05);病变组患者高血压病程、收缩压、舒张压、血清CRP、CRP/ApoA1均高于单纯高血压组(t=7.539、6.192、10.923、24.049、17.362,P<0.05);病变组患者NQO1、sRAGE低于单纯高血压组(t=11.530、6.746,P<0.05);Pearson分析显示血清NQO1、sRAGE与Gensini评分负相关(P<0.001),CRP/ApoA1则与Gensini评分呈正相关(P<0.001),多因素Logistic回归分析显示患者收缩压、舒张压、糖尿病史、血清CRP、ApoA1、NQO1、CRP/ApoA1、sRAGE均是其并发冠状动脉病变的影响因素(P<0.05).结论 原发性高血压患者合并冠状动脉病变其血清NQO1、sRAGE水平与病变程度负相关,CRP/ApoA1与病变程度正相关,可作为高血压患者冠状动脉病变预测指标.
Correlation between serum NQO1,CRP/ApoA1,sRAGE and coronary artery disease in patients with essential hypertension
Objective To investigate the correlation between serum quinone oxidoreductase(NQO1),C-reactive protein to apolipoprotein A1 ratio(CRP/ApoA1)and soluble advanced glycation end-product receptor(sRAGE)and coronary artery disease in patients with essential hypertension.Methods The data of 122 patients with essential hypertension from the First Affiliated Hospital of Guangdong Pharmaceutical University from June 2021 to December 2022 were retrospectively analyzed.Patients with coronary artery dis-ease were divided into two groups:the simple hypertension group(77 cases)and the coronary artery disease group(45 cases)based on whether they were complicated with coronary artery disease.Within the coronary ar-tery disease group,patients were further divided into mild(14 cases),moderate(18 cases)and severe group(13 cases)groups according to the Gensini score.Serum levels of NQO1,CRP/ApoA1,and sRAGE were com-pared between the two groups.The correlations between NQO1,CRP/ApoA1,sRAGE,and Gensini scores were analyzed using the Pearson correlation test.A multivariate logistic regression model was used to analyze the influencing factors of coronary artery disease in hypertensive patients.Results Compared to the simple hy-pertension group,the proportion of smoking and diabetes in the pathological change group was significantly higher(x2=22.215,9.872,P<0.05).The duration of hypertension,systolic blood pressure,diastolic blood pres-sure,serum CRP,and CRP/ApoA1 in the lesion group were higher than those in the simple hypertension group significantly(t=7.539,6.192,10.923,24.049,17.362,P<0.05),while the NQO1 and sRAGE in the lesion group were significantly lower(t=11.530,6.746,P<0.05).Pearson analysis showed that serum NQO1 and sRAGE were negatively correlated with the Gensini score(P<0.001),while CRP/ApoA1 was positively corre-lated with the Gensini score(P<0.001).Multivariate logistic regression analysis showed that systolic blood pres-sure,diastolic blood pressure,diabetes history,serum CRP,ApoA1,NQO1,CRP/ApoA1,and sRAGE were all influential factors for complicated coronary artery disease(P<0.05).Conclusion In patients with essential hypertension complicated by coronary artery disease,serum NQO1 and sRAGE show a negative correlation with the severity of lesions.Conversely,CRP/ApoA1 demonstrates a positive correlation with the lesion sever-ity,suggesting it can be used as a predictor of coronary artery disease in hypertension patients.
