外周血中NLRP3炎症小体及血清IL1β、IL18水平与2型糖尿病合并慢性牙周炎的关系

目的 探究外周血中NOD样受体热蛋白结构域相关蛋白-3(NLRP3)炎症小体及血清白介素-1β(IL-1β)、白介素-18(IL-18)水平与2型糖尿病(T2DM)合并慢性牙周炎的关系.方法 回顾性分析2022年3月至2023年2月石家庄第二医院收治的60例T2DM合并慢性牙周炎患者的临床资料,将其设为治疗组,同时随机选择本院60名健康体检人群,设为对照组,对照组不进行处理,治疗组进行铒激光联合牙周基础治疗.比较两组治疗前NLRP3炎症小体、IL-1β、IL-18.比较治疗组治疗前、治疗1周后、治疗 2周后、治疗 4周后血清炎症水平(NLRP3炎症小体、IL-1β、IL-18)、血糖、牙周情况变化,使用Pearson法分析治疗 4周后NLRP3炎症小体水平与IL-1β、IL-18、FPG、PBG、PD、PLI、AL、BI的相关性.结果 治疗组NLRP3炎症小体、IL-1β、IL-18均高于对照组,差异均有统计学意义(P<0.05);治疗后,治疗组NLRP3炎症小体、IL-1β、IL-18、PD、PLI、AL、BI均低于治疗前,且指标水平随治 疗时间降低,差异均有统计学意义(P<0.05);治疗后,治疗组FPG、PBG与治疗前比较,差异无统计学意义(P>0.05);Pearson法分析结果显示,NLRP3 炎症小体水平与IL-1β、IL-18、FPG、PBG、PD、PLI、AL、BI均呈正相关性(P<0.05).结论 T2DM合并慢性牙周炎患者NLRP3炎症小体、IL-1β、IL-18水平异常升高,NLRP3通路可能促进了T2DM合并慢性牙周炎患者发病过程.
Relationship between peripheral blood NLRP3 inflammasome,serum IL-1β,IL-18 levels and type 2 diabetes mellitus with chronic periodontitis
Objective To investigate the relationship between NOD-like receptor pyrin domain containing 3(NLRP3)inflammasome in peripheral blood,serum interleukin-1β(IL-1β),interleukin-18(IL-18)levels and type 2 diabetes mellitus(T2DM)with chronic periodontitis.Methods The clinical data of 60 patients with T2DM complicated by chronic periodontitis admitted to Shijiazhuang Second Hospital from March 2022 to February 2023 were retrospectively analyzed.These patients were designated as the treatment group.60 healthy people who went through physical examinations at our hospital were randomly selected as the control group.The control group did not receive any treatment,while the observation group underwent erbium laser therapy combined with basic periodontal treatment.The levels of NLRP3 inflammasome,IL-1β,and IL-18 be-tween the two groups before treatment were analyzed.In the treatment group,changes in serum inflammation levels(NLRP3 inflammasome,IL-1β,IL-18),blood glucoselevels,and periodontal conditions were compared before treatment and after 1 week,2 weeks,and 4 weeks of treatment.The Pearson method was used to analyze the correlation between NLRP3 inflammasome level and IL-1β,IL-18,FPG,PBG,PD,PLI,AL,BI after 4 weeks of treatment.Results The levels of NLRP3 inflammasome,IL-1β,and IL-18 in the treatment group were higher than those in the control group(P<0.05).After treatment,NLRP3 inflammasome,IL-1β,IL-18,PD,PLI,AL,and BI in the treatment group were lower than before treatment,and the levels decreased with treatment time(P<0.05).After treatment,there were no statistical differences in FPG and PBG in the treatment group compared with before treatment(P>0.05).Pearson method analysis showed that the level of NLRP3 in-flammasome was positively correlated with IL-1β,IL-18,FPG,PBG,PD,PLI,AL,and BI(P<0.05).Con-clusion The levels of NLRP3 inflammasome,IL-1β,and IL-18 in T2DM patients with chronic periodontitis were found to be abnormally elevated.The NLRP3 pathway may play a role in promoting the pathogenesis of chronic periodontitis in patients with T2DM.
