Relationship between serum IL-6,PCT/ALB,and IL-1β levels and prognosis in elderly patients with severe pneumonia
Objective To investigate the association between the prognosis and blood levels of IL-6,PCT/ALB,and IL-1β in old patients suffering from severe pneumonia(SP).Methods Eighty-eight elderly patients with SP were treated in the Department of Intensive Care Medicine at the Fifth People's Hospital of Kunshan City from January 2021 to January 2024.They were divided into 64 cases in the survival group and 24 cases in the death group based on their outcomes after 28 days.The patients'prognosis was assessed by ana-lyzing their serum levels of IL-6,PCT/ALB,and IL-1β in the two groups.We compared the serum levels of IL-6,PCT/ALB,and IL-1β levels between the two groups,examined the correlation between IL-6,PCT/ALB,and IL-1β and prognosis,conducted unifactorial and multifactorial logistic analyses to identify factors influencing the mortality of elderly SP patients,and used ROC curves to evaluate the prognostic value of se-rum IL-6,PCT/ALB,and IL-1β levels in elderly SP patients.Additionally,we assessed the diagnostic value of these markers.Results The death group had a higher PCT/ALB ratio,IL-6,IL-1β,CRP levels,and APACHE Ⅱ scores than the survival group,and the difference was statistically significant(P<0.05).IL-6,PCT/ALB,and IL-1β were linearly and positively associated with prognosis,as shown by Pearson correlation analysis(P<0.05).Increased APACHE Ⅱ scores and high expression of IL-6,PCT/ALB,IL-1β,and CRP are independent risk factors for death in elderly SP patients as indicated by multifactorial logistic regression analysis(P<0.05).The combination of the three factors and the area under the curve(AUC)of serum IL-6,PCT/ALB,and IL-1β levels for predicting death in patients with severe pneumonia were 0.781,0.729,0.736,and 0.911,respectively(P<0.05).Conclusion The occurrence and development of pneumonia in eldly pa-tients with severe sepsis are closely correlated with serum levels of IL-6,PCT/ALB,and IL-1β.Combined testing of these three markers has significant clinical value in assessing the prognosis and can be used as a pre-dictor of severe outcomes.
