目的 评估芪苈强心胶囊对慢性心力衰竭患者肠道菌群、肿瘤坏死因子α(TNF-α)、白介素 1β(IL-Iβ)、心脏型脂肪酸结合蛋白(H-FABP)和氧化三甲胺(TMAO)水平及心功能的影响.方法 选取淄博市中医医院2021年7月至2023年7月间收治的198例慢性心力衰竭患者,随机分为西药组和芪苈强心组,每组99名.西药组给予西药常规治疗,芪苈强心组在此基础上加用芪苈强心胶囊,治疗8周.比较两组治疗前后症状、肠道菌群及血清TNF-α、IL-Iβ、H-FABP、B型利钠肽(BNP)和N末端B型利钠肽原(NT-proBNP)和TMAO水平.测定左心室射血分数(LVEF),6分钟步行试验(6MWT)测定运动耐力,记录不良反应.结果 治疗8周后,芪苈强心组证候积分较西药组明显降低,差异有统计学意义(P<0.05);芪苈强心组的双歧杆菌、乳杆菌和拟杆菌数量均比西药组高,而大肠杆菌、酵母菌则明显少于西药组,差异有统计学意义(P<0.05);芪苈强心组TNF-α、IL-Iβ、TMAO水平均显著低于西药组,差异有统计学意义(P<0.05);芪苈强心组的LVEF和6MWT水平显著高于西药组且H-FABP、BNP、NT-proBNP水平均显著低于西药组,差异有统计学意义(P<0.05).两组间的毒副作用比较差异无统计学意义(P>0.05).结论 芪苈强心胶囊可明显改善心衰患者肠道微生态、炎症及心功能,有临床价值.
Influence of Qili Qiangxin capsule on intestinal flora,plasma TNF-α,IL-Iβ,H-FABP and TMAO levels and cardiac function in patients with chronic heart failure
Objective To evaluate the influence of Qili Qiangxin capsule on intestinal flora,lev-els of tumor necrosis factor-alpha(TNF-α),interleukin-1 beta(IL-Iβ),heart-type fatty acid binding protein(H-FABP)and trimethylamine N-oxide(TMAO)and cardiac function in patients with chronic heart failure(CHF).Methods From July 2021 to July 2023,198 patients with CHF at Zibo Hospital of Traditional Chi-nese Medicine were selected and randomized into two groups:the Western medicine group and the Qili Qiangxin group,with 99 cases in each group.The Western medicine group received routine western medicine treatment,while the Qili Qiangxin group received Qili Qiangxin capsules in addition to the routine treatment.Both groups underwent an 8-week treatment period.Before and after treatment,symptoms,intestinal flora and levels of serum TNF-α,IL-Iβ,H-FABP,B-type natriuretic peptide(BNP),N-terminal pro-B-type natriuretic peptide(NT-proBNP),and TMAO were compared between groups.The left ventricular ejection fraction(LVEF)was measured,and the 6-minute walking test(6MWT)was used to evaluate exercise tolerance.Ad-verse reactions were aslo recorded.Results After 8 weeks of treatment,the syndrome scores in the Qili Qiangxin group were significantly lower than those in the Western medicine group(P<0.05).The counts of Bi-fidobacterium,Lactobacillus and Bacteroides in the Qili Qiangxin group were higher than those in the Western medicine group,while the counts of Escherichia coli and yeast were significantly lower than those in the West-ern medicine group(P<0.05).The levels of TNF-α,IL-Iβ,and TMAO in the Qili Qiangxin group were sig-nificantly lower than those in the Western medicine group(P<0.05).The levels of LVEF and 6MWT in the Qili Qiangxin group were significantly higher,while the levels of H-FABP,BNP,and NT-proBNP were sig-nificantly lower compared to the Western medicine group(P<0.05).Furthermore,there were no statistical dif-ferences in toxic and side effects between the groups(P>0.05).Conclusion Qili Qiangxin capsule can sig-nificantly improve the intestinal microecology,inflammation and cardiac function of patients with heart fail-ure,making it clinically valuable.
