The significance of serum Vitamin A,25-hydroxyvitamin D,and IgE detection in the clini-cal diagnosis and treatment of pediatric bronchial asthma
Objective To investigate the significance of serum vitamin A(VitA),25-hydroxyvi-tamin D(25(OH)D)and immunoglobulin E(IgE)in the clinical diagnosis and treatment of bronchial asthma in children. Methods This study involved a retrospective analysis.Based on the inclusion and exclusion crite-ria,131 children with bronchial asthma who received treatment at the Children's Respiratory Allergy Clinic of Civil Aviation General Hospital from October 2021 to February 2023 were selected as the observation group.Meanwhile,129 healthy children who underwent physical examinations at our hospital during the same period were chosen as the control group. The serum VitA,25(OH)D and IgE levels of children with different sever-ity of asthma were compared between the control group and the observation group.Based on the assessment of asthma control test,131 children with bronchial asthma were divided into a good control group,a partial con-trol group and an uncontrolled group.Clinical data(such as age, sex, exercise, allergy history and severity of asthma)and laboratory indicators(serum VitA,25(OH)D and IgE levels)of the three groups were com-pared.A multiple ordered logistic regression model was used to analyze the related factors affecting the control level of bronchial asthma. Results The levels of serum VitA and 25(OH)D in the observation group were lower than those in the control group, and the IgE level was higher than that in the control group, the differ-ences were statistically significant(P<0.05).VitA and 25(OH)D levels were compared among the three groups:mild asthma>moderate asthma>severe asthma;IgE levels were compared among the three groups:severe asthma>moderate asthma>mild asthma, and the difference was statistically significant(P<0.05).Out of the 131 children with bronchial asthma,36 cases were well controlled,53 cases were partially controlled, and 42 cases were not controlled. There were no statistically significant differences in age, gender, exercise, and allergy history among the three groups(P>0.05).However,there were statistically significant differences in the severity of the disease and the levels of VitA,25(OH)D and IgE among the three groups(P<0.05).The results of multiple ordered logistics regression analysis showed that when the control level of childhood asthma was taken as the dependent variable, the severity of the disease, the decrease of serum VitA,the de-crease of 25(OH)D and the increase of IgE level were the independent risk factors affecting the control level of childhood asthma(P<0.05).Conclusion The detection of serum VitA,25(OH)D and IgE detection of these three levels is helpful for evaluating the severity of the disease, providing a reference for formulating in-dividualized treatment plans, and judging disease progression.
