摘要
目的 通过对本地区早期原发性肝癌筛查项目数据及推进中的问题分析,阐述基层筛查工作现状及探索"疾控-医院-基层"的互联互动筛查模式,为各地从事肝癌筛查的同道提供一些参考.方法 对滁州本市(南谯区沙河镇与章广镇)农村及市区常住居民,通过肝癌健康科普、义诊、融媒体等多种形式项目宣传;由各村/社区卫生人员采取电子问卷调查初步筛选出具有高危因素人群并获得知情同意后,进入高危人群筛查流程.同时,纳入受检所在地的国家老年人(65岁以上)免费体检筛查数据,一并汇总分析.结果 基层筛查人员缺乏专业培训体系,有效的肿瘤层级筛查及持续监测有待进一步具体落实;建立以本区域医疗单位为中心的"疾控-医院-基层"的互联互动筛查模式,可以有效地统筹优化筛查各环节及汇总分析建模,提高筛查效率减少成本.
Abstract
Objective By analyzing the data from the early screening program for primary liver cancer in this region and the problems that have occurred in the implementation process,this study will explore the current situation of grassroots screening work and the interactive and interconnected screening model of"Disease Control-Hospital-Grassroots",and provide some references for peers who are engaged in liver cancer screening in various regions.Methods For the permanent rural and urban residents in Chuzhou city(Shahe Town and Zhangguang Town in Nanqiao District),various forms of project promotion were carried out through liver cancer health science popularization,free clinic service,converged media,etc.The electronic questionnaire survey was used by the village/community health personnel to initially screen out individuals with high-risk factors and obtain informed consent before entering the high-risk population screening process.At the same time,the data from the free physical examination and screening of national seniors(aged 65 and above)in the tested areas were included and analyzed together.Results The grassroots screening personnel lack a pro-fessional training system,and effective hierarchical tumor screening and continuous monitoring need to be further implemented specific-ally.Establishing an interconnected and interactive screening model centered on local medical units with a focus on"disease control-hospitals-grassroots"can effectively coordinate and optimize all aspects of screening,as well as aggregate,analyze,and model data,thereby improving screening efficiency and reducing costs.
